AIMS OF STUDY • To investigate the quality of life of the patient with a leg ulcer, attending the community leg ulcer clinics in the Midland Health Board. • To ascertain if patients who have attended the community leg ulcer clinics are satisfied with the service provided. OBJECTIVES • To review the literature in relation to quality of life and leg ulcers in order to highlight issues which may be relevant to this group of patients. • To develop a research methodology suitable for this patient group in order to allow patients to explore other quality of life issues which maybe relevant to their situation. • To explore the patients perception of the service provided.

Abstract:
Cleanliness counts Ensuring a clean environment is fundamental to the provision of safe and high quality care. The hospital environment is one of the first things which patients and their families experience when they seek care and ensuring this is clean gets the first impression right. Importantly, some aspects of hygiene services support the prevention and control of healthcare infection. Healthcare infection is a concern for those receive and provide care alike and, while recent downwards trends in MRSA-related bloodstream infection are encouraging, there is no room for complacency. Lessons learned Two key lessons emerged from HIQA’s hygiene service reviews in 2007 and 2008. Firstly, hospitals operated or funded by the HSE are making improvement; fundamentals are in place with services on the ground performing well and hospitals are now developing the corporate management to sustain performance and ensure continuous improvement. Since the last edition of the Ethical Guide in 1989. there have been many changes in the practice of medicine ranging from high technological procedures ethical problems relating to reproductive medicine genetics serious infection advertising and in general. changes in social conditions of many of our patients. The present Guide has been revised and a new format established with numerical identifiable paragraphs to simplify referrals to the Guide. The Ethics Committee has spent a lot of time in the past year re-editing for this issue and I must compliment Professor P. Keane for his enthusiasm and diligence as Chairman of the Committee. New changes relate to data protection. donor organs, protocol on brain death a statement on abortion deputising arrangements. requests for medico-legal reports in-practice information.

Abstract:
Many infections of importance to public health are seen almost exclusively in primary care (e.g. childhood infections, respiratory and diarrhoeal illness). Acute gastroenteritis is a common illness in the community affecting several thousand patients every day with significant morbidity and economic loss, in both Northern Ireland and the Republic of Ireland. However, routine information sources from primary care do not measure the true burden of disease. While clinical notifications from GPs and laboratory reports give some indication of the frequency of the disease, these are known to greatly underestimate the condition. Also, under-reporting of infectious diseases in general practice has been documented.

Abstract:
The Department of Health and Children (DoHC) has issued guidelines for asylum seeker communicable disease screening in Ireland. This screening is offered on a voluntary basis in reception centres in the Eastern Region and follow-up is provided in some areas throughout the country to which asylum seekers are dispersed. The aim of this study was to evaluate and to do a baseline audit of the screening service provided (in reception centres and on dispersal) to asylum seekers who present to the reception centre clinics in relation to the DoHC guidelines. A number of approaches were taken: • interviews with service providers in the Eastern Region; • a survey of service providers in dispersal locations, in the East and in other areas; • analysis and interpretation of asylum seeker communicable disease screening clinic returns made over a number of years from reception centre clinics to Senior Area Medical Officers and the Director of Public Health; and • follow-up of individual screening ...

Abstract:
Tuberculosis (TB) is a preventable infectious disease which, if not controlled, has major public health implications. TB remains a leading infectious cause of death globally, with almost two million deaths per year. Currently, one-third of the world's population is infected with TB. In 1991,640 TB cases were notified in Ireland. The national trend in TB notifications was downward until 2003, when 407 TB cases were notified to the Health Protection Surveillance Centre. Data for Ireland for 2004 and 2005 show that TB notifications are increasing. Four hundred and sixty one TB cases were notified (crude rate11.81l 00,000 population) in 2005. The proportion of TB cases notified amongst non-Irish nationals has risen from 8.3% of notified cases in 1998 to almost one-third of notified cases in 2005. To date no case of extensively-drug resistant TB has been recorded in Ireland but cases of multi-drug resistant TB have been recorded on an almost annual basis since 1999.

Abstract:
A retrospective cohort study was conducted to investigate an outbreak of Norwalk-like viral gastroenteritidis that occurred in Irish holidaymakers visiting Andorra, in January-February 2002. Preliminary results showed the risk exposure was higher for tourists who stayed in Soldeu and consumed ice cubes in their drinks (OR = 2.5, 95% CI [1.3-4.6)], after logistic regression and adjusting for sex and water consumption).
