<p>PURPOSE: The purpose of this paper is to summarise the recent debates and issues on the healthcare system in Ireland, which have come to the fore through media exposure. The implications for these debates on quality are suggested and questions are raised to stimulate further debate.</p> <p>DESIGN/METHODOLOGY/APPROACH: Recent reports and media opinion articles are reviewed in the light of the health reform programme and the increased prosperity due to the Celtic Tiger era in Ireland.</p> <p>FINDINGS: The Health Service in Ireland is not what it should be. <p>Well-trained and motivated medical staff are essential to a functioning health system. However, Ireland is struggling in this regard. As has been highlighted in numerous studies and reports over the years, Irish non-consultant hospital doctors (NCHDs) are demoralised and dissatisfied with working conditions in Ireland and are leaving in increasing numbers. The other side of the coin is Ireland’s growing reliance on overseas-trained doctors, despite the successful scaling-up of numbers of EU (and therefore Irish) entrants to medical schools.</p>

<p>BACKGROUND: The desire of patients for personal continuity of care with a General Practitioner (GP) has been well documented, but not within non-registered private patients in Ireland. This study set out to examine the attitudes and reported behaviours of private fee-paying patients towards continuity of GP care and universal registration for patients.</p> <p>METHODS: Cross-sectional telephone survey of 400 randomly chosen fee-paying patients living within County Dublin. There is no formal system of registration with a GP for these patients. Main outcomes were attendance of respondents at primary health care facilities and their attitudes towards continuity of care and registration with a GP.
