Abstract:
INTRODUCTION: Poor adherence reduces the potential benefits of osteoporosis therapy, lowering gains in bone mineral density resulting in increased risk of fractures. AIM: To compare prescribing and adherence patterns of anti-osteoporotic medications in patients admitted to an urban teaching hospital in Ireland with a fragility type fracture to patients admitted to a rural hospital in the North Western region. METHODOLOGY: We identified all patients >55 years admitted to Sligo General Hospital between 2005 and 2008 with a fragility fracture (N = 744) using the hospital in-patient enquiry system (HIPE). The medical card number of those patients eligible for the primary care reimbursement services scheme (PCRS) facilitated the linkage of the HSE-PCRS scheme database to the HIPE database which enabled a study to identify persistence rates of patients prescribed osteoporosis therapy after discharge. An Bord Altranais was charged by the Minister for Health and Children with responsibility for the development of the regulatory and governance structures to support the introduction and implementation of nurse and midwife prescribing. At the request of the Minister, it was stipulated that the regulations for nurse and midwife prescribing would be evaluated within two years (Department of Health and Children (DoHC) 2007).

Medication reconciliation is a basic principle of good medicines management. With the establishment of the National Acute Medicines Programme in Ireland, medication reconciliation has been mandated for all patients at all transitions of care. The clinical pharmacist is widely credited as the healthcare professional that plays the most critical role in the provision of medication reconciliation services.

Abstract:
Antiepileptic drug (AED) exposure during pregnancy increases the risk of major congenital malformations (MCMs). The magnitude of this risk varies by AED exposure. Here we provide updated results from the UK Epilepsy and Pregnancy Register of the risk of MCMs after monotherapy exposure to valproate, carbamazepine and lamotrigine.

The abuse of non prescription or OTC medicines has yet to be quantified in this country, However the large number of reports received by this Society in relation to the abuse of laxatives, codeine containing products, paracetamol and cough mixtures would suggest that the problem is an ever increasing one. In particular, a recent report' concerning a six-year old child who inadvertently received a moderately small dose of paracetamol in a 48 hour period and required a liver transplant as a result of metabolite induced hepatotoxicity clearly indicates the potential dangers of this universally available drug, Furthermore, the continued availability of paracetamol from non-pharmacy outlets is a source of grave concern to the Pharmaceutical Society of Ireland. Abstract:
Administrative health data, such as pharmacy claims data, present a valuable resource for conducting pharmacoepidemiological and health services research. Often, data are available for whole populations allowing population level analyses. Moreover, their routine collection ensures that the data reflect health care utilisation in the real-world setting compared to data collected in clinical trials.
The Irish Health Service Executive-Primary Care Reimbursement Service (HSE-PCRS) community pharmacy claims database is described. The availability of demographic variables and drug-related information is discussed. The strengths and limitations associated using this database for conducting research are presented, in particular, internal and external validity. Examples of recently conducted research using the HSE-PCRS pharmacy claims database are used to illustrate the breadth of its use.

Abstract:
The National Drugs Advisory Board received 716 reports from medical and dental practitioners during 1975. From these 1180 side-effects were recorded associated with the use of drugs during the year. This figure is almost double the number reported for the previous year, but has been inflated by 233 side-effects reported by dentists consequent to the use of a particular batch of a local anaesthetic. Drugs interactions were reported in 25 instances 2 per cent in most cases involving either tranquillizers or anti hypertensives. The frequency of reporting of such interactions is increasing and many more will undoubtedly be encountered, particularly in those patients on long-term maintenance therapy for such conditions as anxiety neuroses, hypertension and congestive heart failure. It is important that doctors and dentists be aware of all the medications (even those which are self-prescribed) taken by his patient before prescribing additional treatments.
