Abstract:
In 1999, 12,996 people died from vascular diseases in Ireland. In 1950, 11,887 people died from the same group of diseases. The populations of the respective years differed, of course (approximately 4 million vs 2.9 million), and a higher proportion of those who died in 1950 were less than 65 years old. However, it is relevant to ask what has happened in relation to vascular diseases in Ireland in the last 50 years, and what predictions can be made about the occurrence of these diseases in the future. Vascular diseases, of which cardiovascular disease is the most common, account for over 40% of all deaths and 37% of deaths under 65 years in Ireland at this time. Within cardiovascular disease, ischaemic heart disease (IHO) is by far the most common. It alone accounts for approximately 25% of all deaths. Abstract:
Introduction: Mi(cro)RNAs are small non-coding RNAs whose differential expression in tissue has been implicated in the development and progression of many malignancies, including prostate cancer. The discovery of miRNAs in the blood of patients with a variety of malignancies makes them an ideal, novel biomarker for prostate cancer diagnosis. The aim of this study was to identify a unique expression profile of circulating miRNAs in patients with prostate cancer attending a rapid access prostate assessment clinic. Methods: To conduct this study blood and tissue samples were collected from 102 patients (75 with biopsy confirmed cancer and 27 benign samples) following ethical approval and informed consent. Introduction: The economic burden of hearing impairment is an area of increased interest. In this paper we examine the relationship between hearing impairment and service use in 14 European countries. Methods: Based on the Survey of Health Ageing and Retirement in Europe (SHARE) undertaken in 2013, Poisson regression models are used to analyse the relationship between the number of visits/number of nights in hospital, and hearing impairment controlling for a number of covariates. Results: We find that hearing impairment is generally associated with increased use of primary and secondary healthcare services when other aspects of health have been controlled. Comparative analysis revealed that where access to hearing assistive technology was greatest the additional use of services was least. This report presents the findings of research conducted on behalf of the Irish Hospice Foundation, with the support of the Health Services National Partnership Forum, by Weafer and Associates Research & Consultancy Ltd and TNS MRBI. The research has been undertaken as part of the work of the Care for People Dying in Hospitals Project, an Irish Hospice Foundation initiative, which is in partnership with the North-Eastern Health Board at Our Lady of Lourdes Hospital in Drogheda. The principal aim of the research was to explore and understand the attitudes of the Irish population towards death and dying and towards the care of people dying in Irish hospitals.

The present research is part of a larger study into the development of an Ethical Framework for End-of-Life Care and its primary purpose was to explore what Irish people think about diverse end of life issues, including what they understand as autonomy in the context of care.

Background: High rates of mental health disorders have been reported in veterans returning from deployment to Afghanistan (Operation Enduring Freedom: OEF) and Iraq (Operation Iraqi Freedom: OIF); however, less is known about physical health functioning and its temporal course post-deployment. Therefore, our goal is to study physical health functioning in OEF/OIF veterans after deployment. Methods: We analyzed self-reported physical health functioning as physical component summary (PCS) scores on the Veterans version of the Short Form 36 health survey in 679 OEF/OIF veterans clinically evaluated at a post-deployment health clinic. Veterans were stratified into four groups based on time post-deployment: (1Yr) 0 - 365 days; (2Yr) 366 - 730 days; (3Yr) 731 - 1095 days; and (4Yr+) > 1095 days. Abstract:
The CHOIR trial in anemic patients with chronic kidney disease compared epoetin-alfa treatment with low (11.3 g/l) and high (13.5 g/l) hemoglobin targets on the composite end point of death, hospitalization for heart failure, stroke, and myocardial infarction. However, other anemia management trials in patients with chronic kidney disease found there was increased risk when hemoglobin is targeted above 13 g/dl. In this secondary analysis of the CHOIR trial, we compared outcomes among the subgroups of patients with diabetes and heart failure to describe the comparative relationship of treatment to these two different hemoglobin goals. By Cox regression analysis, there was no increased risk associated with the higher hemoglobin target among patients with heart failure. In patients without heart failure, however, the hazard ratio (1.86) associated with the higher target was significant. Accidents and SafetyAlthough the number of deaths caused by accidents has fallen by 37% across all age groups in Ireland over the past twenty years, accidents and falls continue to be relatively common causes of death and injury among older people, often resulting in serious longterm difficulties and even premature death. Consequently, the reduction of accidents in the older population is considered to be an important factor in promoting the general well-being of older people. The 1998 Health Promotion Strategy for Older People, Adding years to life, life to years, which was formulated by the National Council on Ageing and OlderPeople in co-operation with the Department of Health and Children, set a target to reduce the death rate from all accidents and their adverse effects in people aged 65 and over by at least 17% in the year 2005. It also set a target of reducing hospital admissions due to falls in people aged 65 and over by at least 17% in the same year.����

Abstract:
We read with interest Dr Varshney et al.'s article on brief interventions for alcohol use among persons on maintenance treatment published ahead-of-print on August 11, 2015. While alcohol is found in the majority of drug-related deaths that involve illicit drugs, around the globe, maintenance therapy with methadone (MMT), or buprenorphine, reduces morbidity and mortality among people who use opiates. Although one common clinical challenge is comorbid alcohol use and opioid use disorder, with guidelines often recommending withholding methadone in this context given the potential for fatal overdose due to drug interactions, alcohol's impact on the health outcomes of MMT patients has been 'overlooked and underestimated'. Therefore, we examined the impact of heavy alcohol use on mortality among MMT patients.

The mother had to self-identify as an Irish Traveller. The aim of study was to assess the health status of Traveller infants and their mothers, quantify health service use, conditions needing health services and to examine why Traveller infants die.

PURPOSE: the Frailty Index (FI) is a popular operationalization of frailty. FI cut-off points have been proposed to define, regardless of age, frailty categories with increasing risk. Here, an alternative method is described that takes age into account. SUBJECTS AND METHODS: 29,905 participants aged ? 50 from the first wave of the Survey of Health, Ageing and Retirement in Europe. The mean follow-up for mortality was 2.4 years. Curve estimation procedures were carried out between age and a FI, and 50% Confidence Intervals (CI) for the regression mean were derived. As opposed to the usual method (FI ? 0.08: non-frail; FI ? 0.25: frail; rest: pre-frail), the alternative method defines as 'fit for their age' those with a FI below the lower 50% CI; 'frail for their age' those with a FI above the upper 50% CI; the rest as 'average for their age'. Abstract:
Although extreme temperatures have not been identified as a major cause of mortality in Ireland, climate change calls for an evaluation of the past, present and future health risks associated with heat and heat waves. The health impacts of heat were investigated using mortality and temperature data for the period 1981-2003. Data were aggregated in urban areas (Dublin, Cork, Drogheda, Arklow, Dundalk, Galway, Limerick, Waterford and Wexford) and rural areas. Seven heat waves were identified between 1981 and 2003, corresponding to 254 excess deaths (197 in rural areas, and 57 in urban areas). A major episode was observed in rural areas in 1983: +115 [confidence interval CI 95% 96:137] extra deaths between the 5th and the 23rd July 1983. During summer, a 1°C increase above 15°C in the mean temperature was associated with a 1.5% [CI 95% 0.9:2.1] increase in total mortality in rural areas, and a 1.6% [0.6:2.5] increase in total mortality in urban areas. The epigenome is associated with biological factors, such as disease status, and environmental factors, such as smoking, alcohol consumption and body mass index. Although there is a widespread perception that environmental influences on the epigenome are pervasive and profound, there has been little evidence to date in humans with respect to environmental factors that are biologically distal. Here we provide evidence on the associations between epigenetic modifications-in our case, CpG methylation -and educational attainment (EA), a biologically distal environmental factor that is arguably among the most important life-shaping experiences for individuals. In recent history, there has been an increasing concern about population aging and the associated increased economic burden in terms of high health-care expenses and pension payments. The need for decision support tools that can explore population dynamics has become a prominent issue. This study presents a comprehensive framework where one can scrutinize the key demographic drives of fertility (Total Fertility Rate/Age-Specific Fertility Rate) over macroeconomic indicators (technology, education, human capital) under exogenous mortality. The integrated model in this article is developed based on a reformulation of the unified growth theory. The study objectives were to: 1) evaluate the diagnostic performance of passive immunity tests for classification of failure of passive transfer (FPT) risk, based on their relationships with calf health and performance, and 2) describe the epidemiology of morbidity and mortality in suckler beef and dairy calves under Irish conditions. A total of 1392 suckler beef calves (n = 111 farms) and 2090 dairy calves (84 farms) were included in this observational study. Blood samples were collected by jugular venipuncture. Serum samples were analysed for total IgG concentration using an ELISA assay, total protein concentration by clinical analyser (TP – CA), globulin concentration, zinc sulphate turbidity (ZST) units, total solids percentage by Brix refractometer (TS – BRIX), and total protein concentration by digital refractometer (TP – DR). Marine protected areas (MPAs) are attaining increasing importance in the management of marine ecosystems. They are effective for conservation in tropical and subtropical areas (mainly coral and rocky reefs), but it is debated whether they are useful in the management of migratory fish stocks in open temperate regions. World War II created a large marine area within which commercial fishing was prevented for 6 years. Here we analyse scientific trawl data for three important North Sea gadoids, collected between 1928 and 1958. Using statistical models to summarise the data, we demonstrate the potential of MPAs for expediting the recovery of over-exploited fisheries in open temperate regions. Candidaemia is associated with a high mortality. We have reviewed cases of candidaemia over a 2-year period at a tertiary referral hospital in association with the introduction of routine antifungal susceptibility testing. The aim of the study was two fold; firstly to establish the typical profile of a patient who might experience a Candida bloodstream infection and secondly, to evaluate methods of antifungal susceptibility testing. In 2008-2009, 31 patients with candidaemia were retrospectively identified using the Laboratory Information Systems (Apex). Clinical data were obtained by chart review. Antifungal susceptibility testing to fluconazole and voriconazole was carried out on 20 of the clinical isolates using three different methods. These isolates were also sent to the mycology reference laboratory at Bristol and results were compared. The male-to-female ratio was 2.1:1 with an age range from 6 weeks to 89 years. Abstract:
Introduction The Phillips Report on traumatic brain injury (TBI) in Ireland found that injury was more frequent in men and that gender differences were present in childhood. This study determined when gender differences emerge and examined the effect of gender on the mechanism of injury, injury type and severity and outcome. Methods A national prospective, observational study was conducted over a 2-year period. All patients under 17 years of age referred to a neurosurgical service following TBI were included. Data on patient demographics, events surrounding injury, injury type and severity, patient management and outcome were collected from 'on-call' logbooks and neurosurgical admissions records. Results 342 patients were included. Falls were the leading cause of injury for both sexes. Abstract:
Objectives Our aim was to investigate the association of epigenetic age and physical capability in later life. Having a higher epigenetic than chronological age (known as age acceleration (AA)) has been found to be associated with an increased rate of mortality. Similarly, physical capability has been proposed as a marker of ageing due to its consistent associations with mortality. Setting The MRC National Survey of Health and Development (NSHD) cohort study. Participants We used data from 790 women from the NSHD who had DNA methylation data available. Design Epigenetic age was calculated using buccal cell (n=790) and matched blood tissue (n=152) from 790 female NSHD participants. We investigated the association of AA at age 53 with changes in physical capability in women from ages 53 to 60-64.
