'Seeking authorization': a grounded theory exploration of mentors' experiences of assessing nursing students on the borderline of achievement of competence in clinical practice
(2017)
Cassidy, Simon; Coffey, Michael; Murphy, Fiona A.
Abstract:
Aim. To develop a substantive theoretical explanation of how mentors make sense of their experiences where nursing students are on the borderline of achievement of competence in clinical practice.Background. The reluctance of Registered Nurse mentors to fail nursing students in clinical practice despite concerns about competence remains a contemporary issue in international healthcare education. Mentors' assessment decisions have considerable impact for a variety of key stakeholders, not least for students in these circumstances.Design. Grounded theory qualitative study.Methods. Phase one involved 20 individual semi-structured interviews with nurse mentors in one United Kingdom National Health Service Health Board (July-October 2009). Phase two included eight individual semi-structured interviews and seven focus groups with mentors and practice educators (n = 38) in four further Health Boards (June 2011-February 2012). Data were analysed using open, axial and selective coding c...

'Visible-ness': the nature of communication for children admitted to a specialist children's hospital in the Republic of Ireland.
(2008)
MCCARRON, MARY
Abstract:
This article presents the core concept of 'visible-ness' that emerged from an ethnographic study, which explored the nature of communication, for children (for ease of reading children refers to children and young people), admitted to a children's hospital in the Republic of Ireland. BACKGROUND: The importance of engaging with both child and family has been espoused as fundamental in promoting 'family'-centred care. To date, studies have almost exclusively explored parents' and nurses' perspectives of the nature of parent participation and nurse-parent interactions and relationships. Although there is evidence of an emerging body of knowledge, which explores children's perspectives of their information, and communication needs, little is known empirically about the communication process between children and members of the health care team in inpatient hospital settings. DESIGN: The principles of ethnography underpinned the study design. METH...

“A great stress among students” - mental health nurses' views of medication education: a qualitative descriptive study
(2019)
Goodwin, John; Kilty, Caroline; Harman, Mark; Horgan, Aine M.
Abstract:
Background: Registered mental health nurses report dissatisfaction with the level of medication knowledge demonstrated by undergraduate nurses. However, little is known about which particular areas staff nurses are concerned about, and how they feel education can be enhanced in both academic and clinical settings. Objective: To investigate the views of staff nurses on the delivery of medication education to undergraduate mental health nurses. Design: A qualitative descriptive design was adopted. Methods: Two focus groups were held with registered nurses in two acute mental health facilities. Data were analysed using qualitative content analysis. Results: The first theme reports on the difficulties staff nurses observed with both undergraduate and newly qualified nurses around medication. It was noted that these individuals had difficulties interpreting medication charts/Kardexes, and were unable to provide medication-related education to service users. The second theme reports on st...

A case study of emergency department practice: Reframing the care of people who self-harm
(2012)
Kane, Raphaela
Abstract:
This study examines the complexities of Emergency Department (ED) practice with specific reference to the care and experience of people who self-harm. Self-harm is a major source of suffering for individuals and their families and requires a targeted cohesive and appropriate response from health care agencies. Bounded in time and place an embedded case study design (Stake 1995) is used to explore this phenomenon in a real- life context (Yin 2003). Known issues associated with the case have formed the conceptual structure from which multiple sources of data have been utilised to gather evidence. Case analysis incorporates data from patient interviews, patient case notes, staff focus groups and interviews, policy documents and media coverage. A process of directed content analysis involving inductive and deductive category application is undertaken. Data are triangulated in an effort to elicit divergent and convergent issues, culminating in the development of two overarching theme...

Abstract:

The original article is available at www.biomedcentral.com

BACKGROUND: In mental health services what is commonplace across international frontiers is that to prevent aggressive patients from harming themselves, other patients or staff, coercive measures and foremost, violence management strategies are required. There is no agreement, recommendations or direction from the EU on which measures of coercion should be practiced across EU countries, and there is no overall one best practice approach.

METHODS: The project was conceived through an expert group, the European Violence in Psychiatry Research Group (EViPRG). The study aimed to incorporate an EU and multidisciplinary response in the determination of violence management practices and related research and education priorities across 17 European countries. From the EVIPRG members, one member from each country agreed to act as the national project coordinator for their country. Giv...

A grounded theory of the role of the directors of nursing in band one teaching hospitals in Ireland
(2006)
Hogan, Jennifer Margaret
Abstract:
It is commonly believed that concepts of power, authority and influence are synonymous. Luthans (1992) suggests that power and authority are separate but related concepts. He identifies power as the ability to get an individual or group to do something, while authority gives the person attempting to wield the power legitimacy and is actually a source of power. Unlike authority, power is not always nor does it need to be legitimate. Theories of power will be discussed and integrated into the final theory. A grounded theory methodology was used to explore the role of a sample of the Directors of Nursing in the Band One Teaching Hospitals in Ireland. In-depth interviews were held with eleven Directors of Nursing. A grounded theory of 'Powerless Responsibility’ was identified. The level of responsibility for keeping the hospital functioning is not commensurate with the level of influence and/or power that Directors of Nursing exert. Directors of Nursing in Band One Hospitals in I...

National Council for the Professional Developement of Nursing and Midwifery; Northern Ireland Practice and Education Council on Nursing and Midwifery
Abstract:
This guide has been produced to help you prepare details of practice and quality developments that you are, or have been involved in, to share with other colleagues through the medium of the All-Ireland Practice and Quality Development Database. The database is made up of a number of sections or fields. Some of these fields have menus linked to them that you are asked to select from. Others are blank to allow you to type in the specific information that you want to share. This guide contains instructions to help you through the process of putting details about your practice and quality development initiatives on the database. You will find guidance on how to access the database, how to register as a contributor, how to add a project, how to update a project already on the database, how to search the database and who to contact if you need help. NIPEC and the NCNM are committed to sharing good practice and we look forward to receiving your projects.

A multi-method study to determine the effectiveness of, and student attitudes to, online instructional videos for teaching clinical nursing skills.
(2009)

Abstract:
E-learning is regularly promoted in higher education settings as a way of fostering more flexible approaches to learning. It has been argued however that the ‘potential benefits of new information and communication technology instruments in education’ have not been subjected to critical scrutiny (Debande 2004 p.192). This paper outlines a multi-method evaluation of an e-learning innovation designed to teach clinical skills to student nurses. Responding to the challenges of teaching clinical skills to large class sizes, we developed a set of instructional videos for one undergraduate skills-based module, which are now integral to that module and available online to students on a continuous basis. Evaluation suggests that students’ performance outcomes are unchanged. The students view the flexible and self-management aspects of this method of learning positively, with some attitudinal differences between male and female, and mature and non-mature students. However, it is best used t...

A multi-phase study of contemporary policy and practice in determining nursing skill mix in acute hospitals in Ireland.
(2012)
Shannon, Michael
Abstract:
The profession of nursing has undergone significant changes in Ireland in the last decade, within the wider health service reform context. Determining the optimal nursing skill mix for acute hospitals is perceived as important to ensure the efficient use of the nursing workforce in the provision of safe and high quality care. However, the term nursing skill mix, though widely used, is contested, and practices to determine skill mix in acute hospitals in Ireland are poorly understood. The aim of the study was to examine contemporary policy and practice in determining optimal nursing skill mix in acute hospitals in Ireland. The objectives were to analyse current policy approaches used to determine skill mix nationally and internationally, to examine how the term nursing skill mix is understood and used by managers and policy makers in Ireland and to explore nursing roles in contemporary practice. A review of national and international policy was undertaken highlighting that despite ...

Abstract:
Purpose: This paper reports on results of a study to identify education needs of nurses in acute care environments regarding dementia care in Ireland. Method: As part of a multicentre study of dementia prevalence and in-hospital course, a survey was conducted with nurses (n=151) in six acute hospitals in the south of Ireland. Data were analysed using descriptive statistics. Findings: Despite the availability of a national dementia education programme across all care services, 83% of acute hospital nursing staff perceived that they had insufficient training in dementia care, reflecting the known poor uptake of offered dementia education in acute hospitals. Over two thirds of nurses felt that education on dementia specific communication strategies was insufficient or unavailable. Nurses also reported insufficient knowledge with regard to pain assessment in patients with dementia, or alternatives to physical or chemical restraint. This lack of knowledge has implications for quality of ...

A national study exploring knowledge, beliefs and implementation of evidence based practice among nurses, midwives, lecturers and students in the Republic of Ireland
(2020)
Cleary-Holdforth, Joanne
Abstract:
Background: Evidence-based practice (EBP) is a problem-solving approach to healthcare that combines the use of best available evidence, healthcare professionals’ expertise, and patient preferences/values (Melnyk and Fineout-Overholt, 2019). It yields proven benefits for patients, healthcare professionals and organisations alike. However, EBP implementation remains inconsistent among nurses and midwives. Understanding the factors involved here is key to facilitating system-wide integration of EBP. Aim: This study aimed to establish the EBP beliefs, knowledge, and implementation of nurses and midwives, and the organizational culture of their workplaces. Methods: Seven validated EBP questionnaires and one open question were provided to nursing/midwifery lecturers (n=71) and students (n=222) in nine randomly sampled Higher Education Institutions, and clinical nurses and midwives (n=292) in affiliated general hospitals in the Republic of Ireland. Response rates were 22.3%, 19.4% and 7%...

Abstract:
Aim: This study sought to explore the personal experiences of transgender people with Irish mental health services. Background: The transgender community have been identified as an underserved, under?researched community in Ireland and abroad. While there has been a surge in research carried out with the community in recent years, there is little known about the personal experiences of Irish transgender people with mental health services. Method: Interpretative phenomenological analysis was used to inform data collection and analysis of semi?structured interviews carried out with four research participants all identifying as transgender and having experienced accessing Irish mental health services. Results: Three themes emerged: affirmative experiences, non?affirmative experiences and clinician relationship. Conclusion: Lack of information and non?affirmative experiences are contributing to poor clinician?patient relationships with transgender populations and impacting attritio...

A pilot survey of physical activity in men with an intellectual disability
(2013)
McKeon, Michael
Abstract:
People with intellectual disability are reported as a sedentary population with increased risks of poor health due to an inactive and sedentary lifestyle. As the benefits of physical activity are acknowledged, measuring physical activity accurately is important to help identify reasons for low and high physical activity in order to assist and maintain recommended levels for optimal health. This paper reports a pilot study undertaken to validate the use of a physical activity monitor (Sensewear Armband) and the International Physical Activity Questionnaire (IPAQ) as instruments for measuring and exploring physical activity of men with intellectual disability. The design was a one group descriptive study and data were collected over a 7 day period from 17 men. The Sensewear Armband enabled continuous and long-term measurement of 14 objective physical activity metrics. The IPAQ examined details of physical activity reported over the 7 days. Equivalent results were found in both instrum...

Abstract:
Aim: The aim of this research is to measure the impact planned changes to nurse staffing and skill-mix have on patient, nurse and organisational outcomes. Background: It has been highlighted that there are several design limitations in studies that explore the relationship between nurse staffing and patient, nurse and organisational outcomes; not least that the vast majority of research in this area emanates from studies that are predominantly observational in design. There are limited studies that measure nurse, patient, organisational and economic outcomes using a longitudinal design following a planned change in nurse staffing. Design: The research will employ a longitudinal, multimethod approach to evaluate the impact that planned changes in nurse staffing and skill-mix have on wards in three pilot hospitals. Methods: Administrative data collection will take place on a shift-by-shift basis prospectively over a three-year period including the measurement of nursing sensitive outc...
