A Monocular Marker-Free Gait Measurement System
(2010)
Courtney, Jane; De Paor, Annraoi M
Abstract:
This paper presents a new, user-friendly, portable motion capture and gait analysis system for capturing and analyzing human gait, designed as a telemedicine tool to monitor remotely the progress of patients through treatment. The system requires minimal user input and simple single-camera filming (which can be acquired from a basic webcam) making it very accessible to nontechnical, nonclinical personnel. This system can allow gait studies to acquire a much larger data set and allow trained gait analysts to focus their skills on the interpretation phase of gait analysis. The design uses a novel motion capture method derived from spatiotemporal segmentation and model-based tracking. Testing is performed on four monocular, sagittal-view, sample gait videos. Results of modeling, tracking, and analysis stages are presented with standard gait graphs and parameters compared to manually acquired data.

Abstract:

The original article is available at www.imj.ie

Ireland has an ageing population and the elderly are over-represented in Emergency Departments (ED)--a quarter of these presentations resulting from falls. A prospective study design was employed using a convenience sample to profile elderly fallers referred for physiotherapy in ED. Forty-subjects were assessed over 14 weeks. Mean age was 84.88 years (s.d. 7.3) and 31 (77.5%) were female. All demonstrated slow walking speed and 26 (65%) demonstrated poor grip strength. A quarter of subjects reported fear of falling and 30 (75%) were classified as frail. Elderly fallers in ED are a frail group of socially vulnerable patients who demonstrate a risk of further falls. Osteoporosis had been diagnosed in 9 (22.5%) subjects--a low prevalence compared with international research, but 25 (62.5%) subjects had never had a DEXA scan. The prevalence of frailty in ...

BACKGROUND: A research gap exists for optimal management of cervical radiculopathy in the first 12 weeks and short term natural history of the condition is somewhat unclear, although thought to be favourable. The primary aim of this assessor blinded, superiority, 2 parallel group randomised controlled trial is to investigate the effects of a 4 week physiotherapy programme (6-8 sessions) of manual therapy, exercise and upper limb neural unloading tape, compared to a control of weekly phone advice; on disability, pain and selected biopsychosocial measures, in acute and sub-acute cervical radiculopathy patients. A secondary aim is to identify whether any baseline variables, symptom duration or group allocation can predict outcome.

<...

McGrann, Patrick; Keating, Louise
Abstract:

The original article is available at http://www.rcsismj.com/

Abstract Background: There is currently much discussion in the sports medicine literature and mainstream media regarding diagnosing and managing concussion and the appropriate criteria to guide return-to-play decisions. In amateur sport, the chartered physiotherapist is often the primary healthcare professional present at sporting events. At present, there is no research to guide management of the concussed player by physiotherapists, which negatively impacts the consensus of concussion management. Objective: To identify current knowledge and clinical practice patterns (assessment and management) regarding concussion in sport among Irish chartered physiotherapists. Methods: Members of Chartered Physiotherapists in Sports and Exercise Medicine with active email addresses (n=370) were invited to participate in an online cross-sectional survey consisting of four sections: demographics; knowledg...

A System for Monitoring Pressures and Spinal Curvature in Spinally Injured People Immobilised on a Spinal Raft
(2004)
De Fréin, Ruairí; Flinn, Eoin; Burke, Ted
Abstract:
We present a system designed to study the pressure at various ‘hot spots’ on the back of the body and the deformation of the spine experienced by a patient when strapped to a spinal board, and the potential alleviation of both by the addition of an inflatable “spinal raft” (or other similar device). In measuring pressure we devised a system of air-filled sacks interfaced with a PC. Each sack, placed under a particular key point on the body, is inflated until its faces just begin to separate and a switch thereby opens. The pressure reading is then captured and displayed by the computer. Seeking a non-invasive method of measuring the curvature of the vertebral column, we discovered that we could use a magnetometer to measure the vertical displacement of magnetically-tagged vertebrae from a fixed horizontal plane. The results of our study will be employed by an independent body to determine the merits or demerit of the spinal raft.

The objective of this project was to analyse the current access to in-patient stroke services and MDT rehabilitation in an acute stroke centre and to compare these services to the recommended “National Clinical Guidelines and Recommendations for the Care of People with Stroke and TIA” (IHF 2010). A retrospective chart review was carried out, recording activity statistics of all patients admitted with acute stroke over a three-month period. 73 patients (male=40, 54.8%) were included. Patients were discharged from the stroke service after a mean stay of 20.2 days (SD.= 19.3). 76.7% (N=56) of patients were admitted to the acute stroke unit (ASU). The mean length of time from admission to first assessment 3.4 days (SD.=2.68), with an average of 138 minutes of treatment received per day across all disciplines. This is compared to the IHF’s recommendation o...

BACKGROUND: Unilateral peripheral vestibular loss results in gait and balance impairment, dizziness and oscillopsia. Vestibular rehabilitation benefits patients but optimal treatment remains unkown. Virtual reality is an emerging tool in rehabilitation and provides opportunities to improve both outcomes and patient satisfaction with treatment. The Nintendo Wii Fit Plus (R) (NWFP) is a low cost virtual reality system that challenges balance and provides visual and auditory feedback. It may augment the motor learning that is required to improve balance and gait, but no trials to date have investigated efficacy. METHODS: In a single (assessor) blind, two centre randomised controlled superiority trial, 80 patients with unilateral peripheral vestibular loss will be randomised to either conventional or virtual reality based (NWFP) vestibular rehabilitation for 6 weeks.
BACKGROUND: Osteoarthritis (OA) of the hip is a major cause of functional disability and reduced quality of life. Management options aim to reduce pain and improve or maintain physical functioning. Current evidence indicates that therapeutic exercise has a beneficial but short-term effect on pain and disability, with poor long-term benefit. The optimal content, duration and type of exercise are yet to be ascertained. There has been little scientific investigation into the effectiveness of manual therapy in hip OA. Only one randomized controlled trial (RCT) found greater improvements in patient-perceived improvement and physical function with manual therapy, compared to exercise therapy. METHODS AND DESIGN: An assessor-blind multicentre RCT will be undertaken to compare the effect of a combination of manual therapy and exercise therapy, exercise therapy only, and a waiting-li...

Peripheral arterial disease (PAD) is a chronic, progressive disease with significant cardiovascular risk. Symptoms include pain in leg muscles on walking, relieved by rest (intermittent claudication). Treatment aims to maintain or improve quality of life (QoL) by minimising ischaemic symptoms and preventing progression to vascular occlusion. Management strategies include exercise-based interventions. Research from 1989 to 2008 was systematically reviewed to identify the QoL impact of exercise-based interventions in patients with intermittent claudication. Twenty-three studies were identified. Five were randomized controlled trials. Studies were summarized in terms of exercise interventions, QoL measures used and QoL findings. The majority used a generic QoL instrument; most commonly the Short ...

Making Sense of Men's Workout Practices: the Body, Age and Identity
(2008)
Dunne, Andrew
Abstract:
Background: Within sociology it is generally accepted that the body has become an object or “project” that is worked on and transformed as a central part of self-identity (Baudrillard 1998; Corrigan 1997; Featherstone 1991; Giddens 1991; Turner 1995, 1992; Shilling 2003). An alternative to such arguments, Leder (1990) conceptualizes the body as an “absent presence”. He argues that, while the body plays a central role in shaping our experience of the world, we are frequently oblivious to our own bodies. For Leder, bodywork is sporadic. He contends that specific social and/or physiological experiences cause the body to “dys-appear”, or enter into direct consciousness, and that such experiences compel individuals to take action intended to shift the body out of explicit awareness. It has been suggested that these two perspectives are in conflict, because they imply contradictory understandings of the relationship between the body and self-identity. It explores in detail the current needs of people post stroke in Ireland who are living in the community. It also provides a profile of many possible problems that stroke can cause. In addition it provides a comprehensive review of the literature.

Aim

This report contains two distinct pieces of research:

First...
