Abstract:
Since the last edition of the Ethical Guide in 1989, there have been many, changes in the practice of medicine ranging from high technological procedures, ethical problems relating to reproductive medicine, genetics, serious infection, advertising and in general, changes in social conditions of many of our patients. The present Guide has been revised and a new format established with numerical identifiable paragraphs to simplify referrals to the Guide. The Ethics Committee has spent a lot of time in the past year re-editing for this issue and I must compliment Professor P. Keane for his enthusiasm and diligence as, Chairman of the Committee. New changes relate to .data protection, donor organs, protocol on brain death, a statement on abortion, deputising arrangements, requests for medico-Iegal reports, in-practice information.

The database is made up of a number of sections or fields. Some of these fields have menus linked to them that you are asked to select from. Others are blank to allow you to type in the specific information that you want to share. This guide contains instructions to help you through the process of putting details about your practice and quality development initiatives on the database. You will find guidance on how to access the database, how to register as a contributor, how to add a project, how to update a project already on the database, how to search the database and who to contact if you need help. NIPEC and the NCNM are committed to sharing good practice and we look forward to receiving your projects.

Sickness and disability payments make up over 10% of total social welfare expenditure. Over 100.000 people receive income maintenance payments relating to sickness and disability. In this context information about the entitlements of people to sickness and disability payments is clearly essential. This is particularly important in view of the recommendations set out in the recent report of the Commission on the Status of People with Disabilities. This guide focuses on the range of sickness and disability payments which are available from public bodies. primarily the Department of Social Welfare and. to a lesser extent. the Department of Health. Abstract:
Rapid and continuing loss of blood is life-threatening. Ensuring that tissue oxygenation is maintained at a level consistent with avoidance of critical ischaemic organ damage and irreversible organ failure demands speed and skill. The knowledge base for that skill is the subject of this Guideline. Massive haemorrhage does not easily lend itself to clinical trials. Randomised and controlled trials are extremely difficult to design and conduct and blinding of investigators is clearly impossible. Nevertheless several careful studies have been reported that provide a basis for identifying current best practice in massive haemorrhage. Many questions are unanswered, and may continue to be so. For example controversy persists over which fluids to use for initial resuscitation or maintenance of the intravascular volume. Abstract:
Blood transfusion carries a real but unquantifiable risk of adverse outcome in surgical patients from infectious, immunological and cardiorespiratory morbid events. This risk may be reduced, but is not eliminated, by autologous transfusion techniques, which, in turn carry different risk profiles. 2. The only indication for red cell transfusion is to increase the oxygen carrying capacity so as to improve tissue oxygen delivery. 3. No single criterion can be identified as a “trigger for transfusion” because there is no readily available indicator of critical tissue oxygenation. 4. A transfusion is rarely indicated for Hb > 9g/dl. and is almost always indicated for Hb < 6g/dl, (particularly when the anaemia is acute). 5. Each patient should be considered on an individual basis with assessment of the clinical signs and symptoms, particularly those of haemodynamic instability, comorbidity and the risk of further blood loss. 6. Abstract:
Research into homelessness has been predominantly quantitative in design, solution-focused and may have effectively concealed the phenomenon itself. This study utilised a hermeneutic phenomenological methodology, based on Martin Heidegger’s philosophy, to reveal some essential, constitutive characteristics of homelessness. In-depth qualitative interviews were conducted with six participants accessed through a service provider for homeless persons. Passing Time and Taking Care emerged as essential, constitutive characteristics and were examined to deepen our understanding of the meaning and ground of homelessness itself (Heidegger, 1962). Passing Time revealed boredom as the mood of homelessness and disclosed the daily challenges faced in filling or wasting time when homeless. It is revealed that through profound boredom, homeless persons exist as an empty self, suspended in an empty world. Decision support systems are currently achieving higher classification accuracies by using more complex reasoning mechanisms. Examples of such mechanisms include support vector machines and neural networks. However in spite of these increases in accuracy many decision support systems are not accepted by users. In domains where there is a high cost associated with incorrect classifications, such as medical domains, users are not always willing to accept a decision support system's classification without proper justification. In every walk of life, from the home to the workplace, people use explanations all the time to justify their opinions. In a unanimous judgment handed down on 8th May 2007, the Irish Supreme Court upheld an appeal by the Irish League of Credit Unions (ILCU) against a High Court judgment that ILCU had abused a dominant position. The Irish Competition Authority had alleged that ILCU had abused its dominant position in the market for savings protection services (SPS) by limiting access to SPS services to its own members. The Authority claimed that credit unions wishing to obtain SPS services were required to also purchase credit union representation services from ILCU and that such tying amounted to an abuse of dominance by ILCU. This was the first abuse of dominance case brought by the Irish Competition Authority to go to a full hearing, the first to be appealed to the Supreme Court and the first Irish case under EC Regulation 1/2003. The case raised a number of interesting economic issues, which are of interest in the context of the debate on the need for a more economics based approach to Article 82....

Abstract:
A total of 315 species of rotifers are known to occur in Ireland. Of these 71 belong to the Bdelloidea and 244 to the Monogononta. Three species new to Ireland are now recorded and one of them is also a new record for the British Isles. A taxonomic list of the 315 species is given together with notes on ecology, where possible, and distribution. The names of recorders are included.

Interruptions and distractions are significant factors in medication errors in the pharmacy environment. “Although only a small percentage of these errors cause harm, medication errors need to be minimised in an effort to improve patient safety1.” Due to the volume of telephone calls within the Regional Oncology Haematology Pharmacy (ROHP), we decided to analyse all phone calls over a secound three week period , the phonecalls were analysed in the same way using the same time frame and parameters. The recommendations from the previous study were applied . The staff in ROHDU were told which extensions were to be used when directing queries into ROHP. All go ahead confirmations were to be emailed to ROHP as apposed to ring through on the phone.

Abstract:
In a typical IPO game with first-price auctions, we argue that risk-averse investors always underbid in equilibrium because of sucbjective interpreetations of the firm' communications about its actual value and resulting risk aversion about the likelihood of facing investors with higher valuations. We show that the noisier the investors' inferences of the firm' value(in the sense of first-order stochastic dominance) the higher the underbidding level. Our finding is independent of winner's curse effects and possible irrational, and allows for a testable theory.
