<p><strong>Objectives </strong>- This thesis proposes the question, Advance Healthcare Directives – Friend or Foe? Is it a good thing or a bad thing to have an advance healthcare directive?</p> <p><strong>Ethical and legal considerations</strong> - I introduce the concept of an advance healthcare directive and why they developed. They started in the US where they are called ‘living wills’. They aim to promote patient autonomy and self determination in advance of the loss of capacity. Ethical arguments about making decisions for a ‘future self’ have been balanced with legal arguments for PVS patients being allowed to die by refusing treatments. <p>D. Abstract Objectives: To systematically explore the perceptions parents and clinicians have of the informed consent process in neonatal research. Methods: A comprehensive literature review of peer reviewed studies, all of which are related to the topic of this dissertation. Additionally, an original questionnaire based study was conducted. The study recruited parents of newborn infants who had been an inpatient of the Neonatal Intensive Care Unit of the Cork University Maternity Hospital and clinicians currently employed in either a paediatric or neonatal setting. Results: Parent perception of informed consent in neonatal research is a relatively well studied area. <p>The field of stem cell technology has seen an exponential growth over the last decade. Due to this, little attention has been paid as to whether this new technology adequately improves on previous research models, or indeed if it causes any ethical or legal issues itself. Of particular interest is whether Induced Pluripotent Stem Cells resolve the ethical and legal issues which have been generated by Human Embryonic Stem Cells. The ethical issues of note relate to the definition of personhood and moral status, the debate on when these can be ascribed, the consequences of utilising embryos for research, and whether the new method of inducing pluripotency stimulates any ethical issues itself. Having a defined innovation strategy and a formal process are generally found to be associated with superior NPD performance. Innovation is at the top of the business agenda in Ireland but despite its importance, little is known about how Irish organisations manage for innovation; whether they have a strategy or whether formal management processes are used, and with what effect. This study finds that two-thirds of innovation active firms do not have an innovation strategy with even less operating any formal innovation process. Having a more formal innovation process is associated with higher innovation returns; more radical or novel innovations and better exploitation of innovations at the diffusion stage of the innovation value chain. Structuring the innovation process has considerable advantages for small firms; in idea generation where they are more likely to develop ‘new to the market’ ideas; in conversion where they take a more risk taking attitude to investing in radical ideas...

Abstract:
<p>Host defence peptides (HDPs) are short antimicrobial peptides that form a crucial component of the innate immune system. Deficiencies in HDP activity contribute to enhanced susceptibility to infections; this is the case in cystic fibrosis (CF). Exogenously-applied HDPs can compensate for these deficiencies and are attractive as new antiinfectives, but their development as antimicrobials is limited by cytotoxicity. This thesis describes the development of a series of HDP prodrugs whose cytotoxicity is reduced by net-charge decrease. As high levels of the host protease neutrophil elastase (NE) are colocalised in the lung with Pseudomonas aeruginosa infection, the enzyme is the target for prodrug activation. This system limits antimicrobial activity and cytotoxicity to areas infected with the bacterium. <p>The controlled, site specific delivery of therapeutic agents in a minimally invasive manner is an approach that aims to reduce off-target effects and enhance overall efficacy for a variety of disease states. Conversely, the manifestation of off-target effects is correlated with concentrations of therapeutic agents in healthy tissue. These principles have been acknowledged by researchers for a considerable period of time. However, the vast majority of drug and cell delivery is still performed in arguably a suboptimal fashion, which fails to fully exploit these concepts. An alternative is the use of biomaterials for site specific delivery, to enable longer retention of therapeutic agents within the target tissue. <p>Uveal melanoma (UM) is the most common primary intraocular malignancy in adults and 40% develop fatal metastatic disease. Compared to tumours with chromosome 3 disomy, monosomy 3 tumours will nearly exclusively develop metastasis. To identify differentially expressed proteins, quantitative label-free LC-MS proteomic profiling of 8 primary UM tissues from patients with metastasis (M) and 8 from patients without metastasis (NM) was performed. Fifty proteins with ≥ 3 peptides matched and p < 0.05 between the two patient groups were differentially expressed. Thioredoxin-dependant peroxidase reductase (PRDX3) was upregulated and cytosolic non-specific dipeptidase (CNDP2) was downregulated in M compared to NM. Pressure damage has been shown to be preventable in 95% of cases (Chan et al., 2009). The economic and social ramifications of pressure damage are vast. They can drastically impair quality of life and cause considerable suffering for the individual. <p>CF is caused by a mutation in the Cystic Fibrosis Transmembrane conductance Regulator (CFTR) and results in airway surface liquid (ASL) dehydration, impaired muco-ciliary clearance, chronic pulmonary infection and inflammation leading to progressive lung destruction. Specialized Pro-Resolution Mediators (SPM’s) such as Lipoxins and Resolvins are SPM’s involved in the active resolution of inflammation. Previous work showed that Lipoxin A4 restores airway surface liquid height in CF bronchial epithelium. <p>Amyotrophic Lateral Sclerosis is a debilitating, progressive and fatal neurodegenerative disorder resulting from the degeneration and death of motoneurons in the spinal cord and motor cortex which leads to muscle paralysis and death. Neuroinflammation is prominent in ALS pathogenesis, and other neurodegenerative disorders, and is believed to contribute to disease progression. Mutations in the superoxide dismutase 1 gene (SOD1) account for approximately 12 – 20 % of familial ALS cases. Microglia, the resident immune cells of the CNS, are activated concurrent with neuronal injury and mediate mutant SOD1 – induced neurotoxicity thus exacerbating disease progression. Given our wider concerns as to the degree to which the mainstream mass media in Ireland constitutes a public sphere, we have undertaken our analysis in the context of the longstanding debates within media and communication studies focused on how media content shapes public perceptions and beliefs about socially contentious issues. This article presents an analysis of data regarding people’s knowledge of and beliefs about migration into the Republic of Ireland. The data in question has been generated through an intervieweradministered survey conducted with participants primarily in the west, mid-west and south of the country. The findings address such topical issues as common knowledge and popular beliefs regarding: numbers and nationalities entering Ireland; reasons for inward migration; welfare entitlements of asylum seekers and people’s perceptions about the relative ease with which diverse groups integrate. The deployment of both outdoor and indoor Small Cell Base Stations (SCBSs) has attracted significant interest in the wireless industry. However, a critical concern in a large scale deployment is the efficient control of the small cell’s transmission mode. In this paper we propose a load aware approach for dynamic idle mode selection where the load distribution is estimated by the “RF Fingerprints” of the users. Our approach allows pilot signals and most of the processing power of a SCBS to be completely switched off when no active user is in its vicinity, or when the required Quality of Service (QoS) can be provided by the underlying macro base stations. Such an approach significantly reduces the energy consumption of the small cells as well as reducing the pilot pollution and signalling overhead. <p>The aim of this retrospective study was to investigate the use of thoracic Computed Tomography (CT) in the Emergency Department of a Dublin Academic Teaching Hospital over a six month period. Data was retrieved using the hospital's computerised information system. There were 202 referrals in total for thoracic CT from the Emergency Department during this time period. The most common indication for thoracic CT referral was for the investigation of pulmonary embolism with 127 (63%) referrals. There were 40 (25%) referrals for suspected malignancy and lung disease, whilst 8 (4%) of the referrals were for investigation of thoracic aortic dissection, 8 (4%) for infection, and 6 (3%) were for investigation of thoracic injury. Only 8 (4%) of all referrals were for investigation of injury as a result of chest trauma.</p>

<p>Peripheral nerve injury is a life-changing problem, which presents significant therapeutic challenges to clinicians and patients. A number of different approaches to dealing with the issue exist but to date there is no consensus on the most effective method of treatment. With large gap nerve transection or central to peripheral nervous system intersection injury, spontaneous meaningful axonal recovery seldom occurs. Despite surgical intervention with autografts, the functional outcomes are often unfavorable. <p>Student workload (SWL) is one of the main elements of curriculum design, and its measurement is problematic especially in an integrated problem-based-learning (PBL) environment. It is the foundation of determination of the European Credit Transfer and Accumulation System (ECTS) units which are awarded upon students’ achievement. As ECTS depends on the notional, and not on the contact SWL, it has been more flexible for meeting the needs of the modern innovative educational trends. Abstract:
<p>Recent attempts to elucidate the genetic architecture of complex epilepsies have been limited by a variety of issues including phenotypic ambiguities, small sample sizes and restricted genetic scope. This thesis employed a diverse array of genetic mapping and MRI techniques to help improve the power of genetic mapping efforts and thus our understanding of the neurobiological factors contributing towards common forms of the disorder.</p> <p>As part of a collaborative meta-analysis of GWAS data on complex epilepsies, we contributed to the identification of loci at 2q24.3 (implicating the <em>SCN1A</em> gene) and 4p15.1 (implicating the <em>PCDH7</em> gene) as risk factors for epilepsy <em>in general</em>, in addition to a third locus at 2p16.1 (implicating <em>VRK2</em>) as a risk factor for <em>idiopathic generalised</em> epilepsy. Abstract:
The present study investigates how attendees at national celebratory crowd eventsspecifically St. Patrick's Day paradesunderstand the role of such events in representing and uniting the national community. We conducted semi-structured interviews with people who attended St. Patrick's Day parades in either Dublin or Belfast. In year 1, full-length interviews were conducted before and after the events (N=17), and in years 1 and 2, shorter interviews were conducted during the events (year 1 N=170; year 2 N=142). Interview data were analysed using thematic analysis, allowing the identification of three broad themes. The removal efficiency of carbon (C) and nitrogen (N) in constructed wetlands (CWs) is very inconsistent and frequently does not reveal whether the removal processes are due to physical attenuation or whether the different species have been transformed to other reactive forms. Previous research on nutrient removal in CWs did not consider the dynamics of pollution swapping (the increase of one pollutant as a result of a measure introduced to reduce a different pollutant) driven by transformational processes within and around the system. This paper aims to address this knowledge gap by reviewing the biogeochemical dynamics and fate of C and N in CWs and their potential impact on the environment, and by presenting novel ways in which these knowledge gaps may be eliminated. <p>Formative objective structured clinical examination (OSCE) provides an experiential learning activity where students can receive feedback on their clinical skills. Feedback can have a very powerful effect on learning. The aim of the change project was to introduce formative assessment of clinical skills during the clerkship training using OSCE.A pilot formative OSCE was implemented on 17 December 2014.Twenty-five students, nine clinical tutors, and one simulated patient participated in the formative OSCE. The formative OSCE included 10 stations. Students rotated around the stations in groups of two or three. One student performed a clinical task in seven minutes while being observed by the clinical tutor and peers, then a five-minute structured feedback followed. Abstract:
Mary Barber's volume Poems on Several Occasions (1734) was one of the first collections of poetry written by a woman to be published by subscription. The subscription list to this volume, which comprised 918 names -- including a number of illustrious political, literary, and cultural figures -- is a testament to her extensive involvement in the cultural and social circles of Dublin, Tunbridge Wells, Bath, and London during the decade spanning 1725-1735. This thesis holds that Barber's significance lies both, in her achievement as the poet who attracted the most subscriptions from the widest possible range of supporters in the first half of the eighteenth century and, in her development of sophisticated strategies to foster patronage relationships and garner subscriptions. By interrogating Barber's patronage relationships and subscription practices this thesis provides a more complete picture of her literary activities. Abstract:
Sleep apnoea (OSAS) is a multisystem disorder. There is a high prevalence of cardiovascular and metabolic morbidities in patients investigated for sleep apnoea. We aim to evaluate any association between cardiovascular, metabolic and pulmonary co morbidities in patients investigated for OSAS and whether clinical findings based on Epworth sleep score (ESS) and snoring helps in diagnosing sleep apnoea. 258 consecutive patients who were electively admitted for sleep assessment in Peamount Hospital, Dublin from Sept 2009 to Aug 2011 were retrospectively reviewed. 139/258 were diagnosed as OSAS. Cardiovascular, metabolic and pulmonary co morbidities were 46.12%, 37.2% and 29% respectively. There is no correlation found between ESS, Snoring with Apnoea Hypopnoea Index in OSAS group. Screening for OSAS should be considered in patients with certain cardiovascular and metabolic disorders. <p>It is evident from the literature that foot problems can lead to discomfort, pain, ulceration and increased risk of falling in older people. Good and timely foot care can help individuals remain independent, active and mobile and, more importantly, prevent footwear related falls in older people. According to Age Action Ireland, the population of Ireland is ageing rapidly and the number of people aged 65 and over will rise from 532,000 in 2011 to almost 1.4 million by 2046. The Health Service Executive currently provides a range of foot care services for older people but these services are not well developed or standardised nationally.
