Living in Critical Times: The Impact of Critical Incidents on Frontline Ambulance Personnel: A Qualitative Perspective
(2008)
Gallagher, Sharon; McGilloway, Sinéad
Abstract:
Little is known about the impact of Critical Incidents (CIs) on the lives of ambulance personnel. One-to-one interviews were conducted with 27 participants who had experienced CIs during the previous 12 months in order to: assess the nature and impact of CIs on health and well-being; examine attitudes toward support services; and explore barriers to service use. The results showed that incidents involving children, suicides, and grotesque mutilation were the most distressing. Participants reported a wide range of physical and mental health problems including sleep difficulties, angry outbursts, irrationality and feelings of alienation. Key themes included: low support service uptake due to fears relating to confidentiality and machismo; a perceived lack of concern and support from management; and a need for professional counselling and stress awareness training. Emergency Medical Controllers (EMCs) also reported a number of difficulties unique to their role. The findings suggest tha...

St Vincent's Hospital
Abstract:
I wish, however, to focus attention on one major central theme which is fundamental for the effective development of St. Vincent's Hospital. This is the financial and investment requirement for the successful development of St. Vincent's Hospital in the time ahead so that it may continue to : deliver high quality health services in the best interests of its patients and their families ; develop as a premier teaching hospital with its medical school as an essential and important part of the Medical Faculty of University College, Dublin; and contribute to, and reflect in its operations public policy for health. It is critical that the financial and investment requirements for these purposes are met adequately and, of course, as a corollary to this, it is also critical that St. Vincent's Hospital should manage its affairs in a financially prudent, cost-effective and responsible manner.

Abstract:
Patients who use our services have a right to good care and to kind and compassionate treatment. They also have a right to expect that the healthcare professionals who provide that care and the system they work in do their best, in every sense of that term, to deliver high quality care. High quality care means care which is evidence based, appropriate, timely, efficient, effective and patient-centred. It implies that, even when things go wrong, the professionals and the system will do the right thing. It is the action or inaction of senior responsible medical and nursing/midwifery staff in the immediate aftermath of events such as those that are the subject of this Report that make all the difference to effective management. It is vital to patient, public and staff confidence and morale that at the most challenging of times, the healthcare system performs to its highest standard. It is imperative, therefore, that we continue to strengthen policy and practice in respect of patient sa...

DUNDRUM Restriction-Intrusion of Liberty Ladders (DRILL) Audit Toolkit

Abstract:
This series of rating 'ladders' is intended to allow a quantitative and qualitative analysis of the use of restrictive and intrusive interventions as part of the therapeutic management of violence and aggression in psychiatric hospital settings. This is an evolving handbook. The ladders are currently organised to facilitate a behavioural analysis. Context, antecedents, behaviour, interventions, consequences are conceptualised as a series of events organised in temporal sequence so that causes, interactions and effects can be considered. The complexity of analysis possible is limited by the statistical power of the numbers of cases and events available. The use of the DUNDRUM-1 triage security scale may enable some benchmarking of patient groups according to their average need for therapeutic security for comparative purposes, when combined with measures of risk such as the DASA and HCR-20, while the quantitative relational security in a given unit provides an essential f...

Managerial Practices to Ensure the Well-being of Humanitarian Volunteers: A Realist Evaluation
(2020)
Aldamman, Kinan
Abstract:
Background: Humanitarian aid workers are at an increased risk of adverse mental health outcomes due to the nature of their work. Research on the mental health outcomes of volunteer humanitarian workers, who constitute the majority of humanitarian workers worldwide however, is scare with some evidence suggesting that they are more vulnerable to psychological morbidity compared to their paid counterparts. While this may be partially explained by the fact that volunteer humanitarian workers come from the same affected communities that they serve, volunteers also receive fewer resources in terms of security, insurance, support structures, and access to services typically reserved for paid staff. Moreover, as non-professionals, volunteers tend to be less prepared and trained to cope with the challenges of humanitarian work. Furthermore, while the act of volunteering is typically linked with positive outcomes, this relationship tends to be reversed within humanitarian contexts. The litera...

National Maternity Hospital (NMH)
Abstract:
The year marked the end of Dr. Peter Boylan's Mastership and I would like to thank him for his leadership over the past seven years during which time the hospital experienced a significant increase in activity. The Executive Committee express their appreciation of his input and many achievements including the appointment of the current Matron and Secretary / Manager, the development of the Merrion Wing and his focus on communication and openness. During the year the three Dublin Maternity Hospitals engaged Mr. David Kennedy to prepare a Strategic Review Report which addresses the role of the hospitals in the organisation and the delivery of services in the Dublin region. The report, presented to the Department of Health in December, is primarily concerned with the totality of the services provided by the three hospitals in the greater Dublin area including their national tertiary care responsibilities . Its focus is on the development of these services tor the future in a coher...

Caring for patients with Suicidal Behaviour: an Exploratory Study
(2007)
DOYLE, LOUISE
Abstract:
Presentation to the Emergency Department (ED) of patients with suicidal behaviours is relatively common. While many of these patients may be referred on to specialist mental health services, many are either discharged with no psychiatric follow-up or leave before being seen. There is therefore an increasing onus on the staff of Emergency Departments to become involved in the assessment and initial management of this patient group. The aim of this study was to describe the experiences and challenges that nurses encounter when caring for patients who present to the ED with suicidal behaviour. Forty-two ED nurses completed a 15-item semi-structured questionnaire. Participants in this study identified risk assessment as part of their role but did not focus on psychosocial assessment or psychological management of this patient group. Feelings of sympathy and compassion were reported towards these patients; however, there was often a prior judgement of the perceived ?genuineness? of the p...

Radiation Therapists role in support patients in Radiation Oncology Departments
(2009)
MILLER, CARAGH
Abstract:
peer-reviewed
Despite advances in medical technology, cancer is still one of the leading causes of death globally, leaving many patients to deal with the emotional and psychological aspects associated with cancer and its treatment (Department of Children & Health 2006). Women diagnosed with breast cancer have a 70-90% survival rate at 5 years, of which 30% will experience psychological morbidity (Southern Health Board 2002). Hence the prevalence of psychological morbidity needs to be addressed if a holistic approach to healthcare is to be provided. The recognition and management of psychological conditions are an integral part of comprehensive cancer care. As a result, the Health Services Executive as part of the continuing expansion of Cork Radiation Oncology Department created Ireland?s first role of Information and Support Radiation Therapist and many other departments have since followed in their lead. The Radiation Therapist appointed to this role in June 2005 was Ca...

The neglected shadow: European perspectives on emotional supports for early school leaving prevention
(2011)
Downes, Paul
Abstract:
Recent OECD reports emphasise ten key steps to equity in education, with concrete targets related to low attainment and early school leaving. Such steps, however, neglect the importance of emotional dimensions to early school leaving and the consequent need for system level emotional supports. The current study involves qualitative research interviews with senior government officials and secondary school management representatives across eight European countries, with a particular focus on school climate and emotional support issues. Issues raised by interviewees for students at risk of early school leaving include supports for withdrawn children, for those at risk of suicide and those being bullied at school affecting their nonattendance. Other emerging themes include alternatives to suspension and teacher education for improving their conflict resolution skills. Some interviewees explicitly observe the dearth of emotional support services available in practice in their countries. ...

James Connolly Memorial Hospital annual report 1994

James Connolly Memorial Hospital
Abstract:
Hospital Activity 1994 was again a very busy year for the hospital with service activity increasing in many areas. Demands for in-patient services were particularly high throughout the year which resulted in our bed occupancy reaching 100%. This occupancy was achieved through the use of additional beds in the wards. This practice places great strain and demands on patients and staff alike. Staff are to be complimented for their dedication and commitment while working under extremely difficult conditions. The Hospital Management recognises that the primary function of the hospital is the provision of an Accident & Emergency Service and in meeting this commitment the hospital found it necessary on an on-going basis to cancel elective/planned admissions.

Book review of D. Wiger (1997). The clinical documentation sourcebook: A comprehensive collection of mental health records of practice forms, handouts and records. Chichester: Wiley

Carr, Alan
Abstract:
This clinical documentation source book was written to meet the need of North American mental health practitioners for a system of documenting their work within the context of managed care systems. Within such management care systems in order to be reimbursed practitioners must show that services are necessary, and that the assessment and treatment procedures are appropriate to the disability and level of impairment shown by clients. The impact of treatment on the clients' level of functioning must be regularly documented and specific goals and criteria for discharge must be specified.

Predicting Mortgage Arrears: An Investigation Into the Predictive Capability of Customer Spending Patterns
(2014)
Roche, Jamie
Abstract:
The management of credit risk and mortgage arrears has become a very important area in financial services and banking. This dissertation set out to build a statistical model, which incorporates customer spending habits and the current equity value of a property, capable of predicting arrears. Current literature identifies many themes such as negative equity and unemployment that are common occurring factors in mortgage arrears but a multi-faceted approach was required to build a model capable of accurately predicting arrears. Property equity values were included in the model by taking the current outstanding value of the loans and using a property price index to work out the current market value of the property. Transactional data was included in the model as an indicator of the spending habits and trends of the borrowers by deriving monthly values for savings, discretionary spend, necessary living expenses and mortgage payments to give an indication of their overall financial healt...

A survey of biosecurity-related practices, opinions and communications across dairy farm veterinarians and advisors
(2014)
Sayers, Riona; Good, Margaret; Sayers, Gearoid
Abstract:
Biosecurity at farm-level can often be poorly implemented, and lack of information has been cited by many studies as a potential explanation. Veterinary practitioners (VPs) and dairy advisors (DAs) play a central role in the provision of animal health and management services to dairy farmers. The objective of this study was to document and compare biosecurity-related practices and opinions across VPs and DAs in Ireland. A selection of veterinary experts (VEs) from outside of Ireland was also surveyed. Questionnaires were completed and response rates of 47% (VPs), 97% (DAs), and 65% (VEs) were achieved. Significant differences were identified in the promotion and implementation of biosecurity between VPs and DAs, with a higher proportion of VPs regularly receiving requests from (P = 0.004), and dispensing advice to (P < 0.0001), their farm clients. Communication between DAs and VPs was sub-optimal with over 60% of each group not in regular communication with each other. With rega...
