Tracey, Marsha L; Gilmartin, Michael; O'Neill, Kate; Fitzgerald, Anthony P; McHugh, Sheena M; Buckley, Claire M; Canavan, Ronan J; Kearney, Patricia M
Abstract:

The original article is available at www.biomedcentral.com

BACKGROUND: Accurate estimates of the burden of diabetes are essential for future planning and evaluation of services. In Ireland, there is no diabetes register and prevalence estimates vary. The aim of this review was to systematically identify and review studies reporting the prevalence of diabetes and complications among adults in Ireland between 1998 and 2015 and to examine trends in prevalence over time.

METHODS: A systematic literature search was carried out using PubMed and Embase. Diabetes prevalence estimates were pooled by random-effects meta-analysis. Poisson regression was carried out using data from four nationally representative studies to calculate prevalence rates of doctor diagnosed diabetes between 1998 and 2015 and was also used to assess whether the rate of doctor diagnosed diabetes changed over time.

RESULTS: Fifteen studies (eight dia...

Abstract:
Research Strategy for Nursing and Midwifery in Ireland The Report of the Commission on Nursing (1998, para. 6.77) recommended that the Nursing Policy Division in the Department of Health and Children, in consultation with the appropriate bodies, draw up a national strategy for nursing and midwifery research. In response to the above, the Chief Nursing Officer at the Department of Health and Children convened a consultative committee to prepare a research strategy for nursing and midwifery in Ireland. This committee was representative of those with a core interest in nursing and midwifery research. Click here to download PDF 501kb

National Rare Disease Plan for Ireland 2014-2018
(2014)
Department of Health (Ireland)
Abstract:
This is a generic policy framework for rare diseases. Its scope is broad and it applies to all rare diseases, which can number up to 8,000 diseases affecting millions of EU citizens. This policy framework envisages a combined approach with our EU partners and Northern Ireland to diagnose and treat people with rare diseases. We must deepen links with facilities and institutions in other countries where specialist services are available that may be absent in Ireland. The plan elaborates on Ireland’s participation in European Reference Networks, which is the networking of knowledge and expertise through reference centres and teams of experts. These links are emphasized in the report to address the care of patients with rare diseases at both national and European levels. Download the report here

Newborn screening for congenital toxoplasmosis in Ireland
(2016)
Ferguson, Wendy
Abstract:

Introduction: The majority of infants with congenital toxoplasmosis (CT) in Europe are asymptomatic and remain undetected during routine postnatal care. Postnatal screening for CT facilitates early diagnosis, targeted therapy and intervention to optimise neurodevelopmental outcome. A previous study in Ireland demonstrated that maternal toxoplasma seroprevalence was 25%, which implied that 75% remained susceptible to seroconversion in pregnancy. Thus funding was sought for a study to determine 1) the feasibility of postnatal screening for CT, 2) the incidence of CT and 3) outcome during the first decade of life.

Methods: A two-year pilot newborn screening programme was initiated on July 1st 2005. A two-step IgM screening protocol was employed (AutoDELFIA and ISAGA assays) to test dried heel blood spots obtained 72-120 hours after birth. Diagnosis was confirmed using paired mother/infant serology. De...

A Strategy for Maternity Care in Northern Ireland 2012-2018
(2012)
Department of Health (Northern Ireland)
Abstract:
DHSSPS has carried out a review of policy on maternity service provision in Northern Ireland. The review focused on the best available evidence for the care and treatment of mothers-to-be; quality, safety and service sustainability; wider workforce issues; and professional roles and responsibilities. As a result, a draft strategy was developed, which was the subject of a public consultation from 28 September 2011 to 23 January 2012. Following an analysis of the responses to the consultation, the final strategy, A Strategy for Maternity Care in Northern Ireland 2012-2018, was published on 2 July 2012. The Strategy is outcomes focused. The six outcomes are:- give every baby and family the best start in life; effective communication and high-quality maternity care; healthier women at the start of pregnancy (preconception care); effective, locally accessible, antenatal care and a positive experience for prospective parents; safe labour and birth (intrapartum) care with improved experie...

Consultation on the development of an Obesity Policy and Action Plan for Ireland
(2015)
Institute of Public Health in Ireland (IPH)
Abstract:
A report developed for the Department of Health by the institute of Public Health in Ireland. This report presents a summary of the feedback of key stakeholders based on a consultation document presenting options for action to address Ireland’s overweight and obesity epidemic. There was a very significant level of engagement by a wide range of stakeholders which has resulted in a useful set of considerations for policy development. The findings from this consultation strand along with those from the other two strands of consultation will be studied to inform and enrich the forthcoming Obesity Policy and Action Plan.

A National Study of Wellbeing of Hospital Doctors in Ireland
(2016)
Hayes, Blanaid
Abstract:

The working environment for hospital doctors in Ireland has undergone radical change in recent years with hospital posts becoming unattractive to doctors in training and to consultants. For young medical graduates, the tensions between training requirements and service demands have contributed to a ‘brain drain’ with over half leaving to work abroad after graduation. Many consultant posts are vacant or are filled on a temporary basis, impacting on the quality of patient care.

This study sets out to fill a gap in the Irish literature by exploring the topic of wellbeing in hospital doctors. It is a mixed method study using both qualitative and quantitative techniques.

The first research question (what are the most significant workplace stressors to which hospital doctors in the Irish public hospital setting are exposed?) was explored using the Delphi methodology. This allowed individual opinion from a geographically dispersed population t...

Sepsis Management National Clinical Guideline No. 6
(2015)
Department of Health (Ireland)
Abstract:
27.5.2015 Sepsis, severe sepsis and septic shock are used to describe the systemic inflammatory response of patients to infection, as a continuum of progressive and life threatening severity. Sepsis is the clinical syndrome defined by the presence of both infection and a systemic inflammatory response. The aim of this guideline is to facilitate the early recognition and appropriate treatment of sepsis in Ireland in order to maximise survival opportunity and minimise the burden of chronic sequelae. The Guideline Development Group recommends the Surviving Sepsis Campaign Guidelines and the Sepsis 6 bundle as the guide to the management of sepsis in Ireland. 20.7.2016 Medicines play a key role in improving the health of patients in Ireland. Securing timely access to medicines for patients at an affordable price, in particular innovative medicines offering enhanced health outcomes, is a key priority for Ireland. Securing the cost effective and economic provision of medicines to the health services in Ireland is vital to free up resources for continued investment in new and innovative medicines for patients; this Agreement is a key element in delivering that objective. This will ensure that Ireland remains at the forefront of its European peers in terms of early access to these new medicines in an affordable manner and within available resources. Framework Agreement on the Supply and Pricing of Medicines

Abstract:

The original article is available at www.imj.ie

Fetal Alcohol Spectrum Disorders (FASDs) are one of the major causes of preventable developmental delay. There is no register of children with FASDs in Ireland. Up to 81% of Irish women report drinking alcohol during the periconceptual period or pregnancy. We aimed to evaluate self-reported knowledge and practice of doctors working in paediatrics in Ireland with regards to FASDs and alcohol consumption during pregnancy. We circulated a survey to all paediatric doctors in Ireland, either enrolled in specialist training or registered as trainers. Fifty-six respondents (31.3%) were unaware of the existence of FASDs. Sixty-two (34.6%) believed most patients with FASDs have dysmorphic features. Seventy-three respondents (40.8%) routinely ask about maternal alcohol consumption during pregnancy in the context of developmental delay. Thirty-one respondents (17.3%) stated that mild alcohol intake in the third tri...

Communicable disease screening for asylum seekers and refugees in Ireland: an analysis of professional stakeholders’ insights into knowledge into action
(2014)

Abstract:

Introduction

Numerous countries employ migrant communicable disease screening programmes and initial health assessments (IHAs). Although the evidence base under-pinning screening policies has been criticised, relatively little is known about the extent to which screening policies, guidelines and programmes are implemented and practice is evidence-informed.

Methods

A case study design was employed to examine the development of communicable disease screening guidelines for asylum seekers and refugees in Ireland and the enacting of the guidelines and programme in practice. A national mapping study of screening service provision was undertaken, alongside a document review. Three sets of in-depth interviews were conducted with key informants involved in the guideline development, the related broader policy context, either current or pas...

Medical workforce stressors continue to undermine Ireland’s ability to achieve medical workforce sustainability and compliance with the WHO Global Code on the International Recruitment of health personnel. These include: i) high rates of emigration among graduates of Irish medical schools, attracted by working conditions, training and career opportunities in other English speaking countries; ii) the need to be compliant with the European Working Time Directive, which restricts hospital doctors’ working week; and iii) increasing demand.

The result is that the increased domestic supply of doctors is not sufficient to keep Irish hospitals staffed, whic...
