<p>BACKGROUND: Despite the recognition of power as being central to health research collaborations between high income countries and low and middle income countries, there has been insufficient detailed analysis of power within these partnerships. The politics of research in the global south is often considered outside of the remit of research ethics. This article reports on an analysis of power in north-south public health research, using Zambia as a case study.</p> <p>METHODS: Primary data were collected in 2011/2012, through 53 in-depth interviews with: Zambian researchers (n = 20), Zambian national stakeholders (n = 8) and northern researchers who had been involved in public health research collaborations involving Zambia and the global north (n = 25). <p><strong>Aims:</strong> It was planned that the organizational decision makers would have an increased ability to make an informed decision by having an additional source of information by the introduction of the KPI’s. A secondary aim was that by implementing a new idea and amending the knowledge to the organizational context that organizational learning will occur for participates involved in the implementation process. Rationale: This paper details the implementation of a set of Key performance indicators (KPI) at two mental health day hospitals. A set of KPI’s is a tool used to provide core measurements of service functioning and well-being. <p>This thesis consists of 6 work-packages (WPs) that describe the Emergency Department (ED) management of cellulitis; it’s aetiological epidemiology, and risk factors for antibiotic treatment failure.</p> <p>In order to provide a context for this thesis, I performed a scientometric analysis of research output related to skin and soft tissue infections (SSTIs) between 1945 and 2014. Overall, research output was low but increased as a result of the community associated methicillin-resistant <em>Staphylococcus aureus</em> (CA-MRSA) epidemic in North American settings.</p> <p>My observational research work began with two studies describing the ED incidence of cellulitis, antibiotic prescribing practices and physician adherence to cellulitis treatment guidelines. <p>Little is known about associations between the social environment and risk for psychosis within rural settings. This study sought to investigate whether such associations exist within an essentially rural context using a prospective dataset of unusual epidemiological completeness. Using the Cavan-Monaghan First Episode Psychosis Study database of people aged 16 years and older, both ecological analyses and multilevel modelling were applied to investigate associations between incidence of psychosis by both place at onset and place at birth and socio-environmental risk factors of material deprivation, social fragmentation and urban-rural classification across electoral divisions. <p>Background</p> <p>Up to 75% of cases of pharyngitis are inappropriately prescribed antibiotics.<em> </em>In those with a bacterial infection, one strain could be treated with antibiotics, <em>Group A β-Haemolytic Streptococci </em>(GABHS), to minimise complications. The Centor Criteria, a clinical prediction rule, is recommended by UK and Irish General Practitioner guidelines for use in primary care. The Infectious Diseases Society of America recommends the use of RADT in combination with the Centor Criteria, to improve antibiotic prescribing. <p><strong>Background:</strong> There is a higher prevalence of smoking among individuals with mental health difficulties (MHDs) internationally. This thesis aimed to establish impact of smoking on those with MHDs in Ireland in terms of smoking-related diseases and to evaluate current cessation care in both secondary and community mental health (MH) settings, using three interrelated studies guided by the Medical Research Council framework.</p> <p><strong>Methods:</strong> Study 1, a cross-sectional retrospective secondary analysis (n=8,175), established the prevalence of smoking and smoking-related disease in a nationally representative sample of community living adults aged 50 and over. <p>Huntington’s Disease (HD) is rare neuro-degenerative disease that affects over 700 people in Ireland with a further 9,000 requiring information, support and care. Given the rarity of HD it appears that more education for healthcare professionals is needed. Without this education, people with HD may not receive the high quality care they require. Using the DMAIC framework for quality improvement (QI), this project investigated the need for HD education for healthcare staff in a voluntary mental health service in Dublin. Utilising QI tools such as driver and swim lane diagrams, a fishbone diagram and staff survey, the primary drivers leading to lack of specific HD education were identified. The result of the staff survey highlighted that 92% (N=25) of respondents ‘Strongly agreed’ and ‘Agree’ that, there is a need for HD education for staff and only 24% ‘Strongly agreed’ that they received appropriate HD training and education for caring for patients with HD. In particular, questions persist about the role MOOCs will play in the future of formal accredited learning. <p><strong>Background</strong></p> <p>The aim of this thesis was to develop an explicit list of potentially serious alcohol medication interactions for use in older adults and to determine the prevalence and magnitude of risk associated with exposure to these interactions. <p>This dissertation traces the development and implementation of a Quality Improvement Plan (QIP) in the area of Service User Involvement within the mental health department of a paediatric hospital. The change management project described focused specifically on the setting up and initial meetings of two Service User panels – one for parents/carers and their representatives and one for young people and their representatives. The dissertation sets out the rationale for this change project drawing on a number of key health policy publications of the last decade and on the literature base generally in this particular policy area. The HSE change model was used as a framework for the design and implementation of the change project. <p>Change management is now considered a core managerial competency. Under the fiscal and regulatory environment that currently applies, healthcare managers and professionals must be competent in initiating, guiding and managing change to drive efficient and effective delivery of care.</p> <p>This dissertation describes a successful journey through a planned change effort. An evidence-based approach to monitoring adverse effects of clozapine was implemented in a Community (Adult) Mental health Team. Clozapine has the sole license for treatment-resistant schizophrenia, but has been associated with several potentially life-threatening adverse effects. <p>Background:</p> <p>Between 2000 and 2010, Ireland became increasingly dependent on foreign-trained doctors to staff its health system. An inability to train and retain sufficient doctors to meet demand is the primary reason for the dependence on foreign-trained doctors. <p>BACKGROUND: Considerable attention has been given by policy makers and researchers to the human resources for health crisis in Africa. However, little attention has been paid to quantifying health facility-level trends in health worker numbers, distribution and workload, despite growing demands on health workers due to the availability of new funds for HIV/AIDS control scale-up. This study analyses and reports trends in HIV and non-HIV ambulatory service workloads on clinical staff in urban and rural district level facilities. <p>BACKGROUND: Much of the debate as to whether or not the scaling up of HIV service delivery in Africa benefits non-HIV priority services has focused on the use of nationally aggregated data. This paper analyses and presents routine health facility record data to show trend correlations across priority services. METHODS: Review of district office and health facility client records for 39 health facilities in three districts of Zambia, covering four consecutive years (2004-07). Intra-facility analyses were conducted, service and coverage trends assessed and rank correlations between services measured to compare service trends within facilities. RESULTS: VCT, ART and PMTCT client numbers and coverage levels increased rapidly. <p>BACKGROUND: In this paper, we give an overview of methadone treatment in Ireland and outline the rationale for designing an electronic health record (EHR) with extensibility, interoperability and decision support functionality. Incorporating several international standards, a conceptual model applying a problem orientated approach in a hierarchical structure has been proposed for building the EHR.</p> <p>METHODS: A set of archetypes has been designed in line with the current best practice and clinical guidelines which guide the information-gathering process.
