Developing an evidence-based practice healthcare lens for the SCONUL Seven Pillars of Information Literacy model
(2013)
Dalton, Michelle
Abstract:
The SCONUL Seven Pillars of Information Literacy model was revised in 2011 to reflect the interpretation of information literacy in today’s environment. Subsequently, a number of lenses have been developed to adapt the core model to different contexts and user groups. This study develops a lens that aims to reflect the unique information landscape and needs of evidence based practice (EBP) in healthcare. Healthcare professionals across medicine, nursing and allied health disciplines were interviewed to explore their understanding and awareness of the clinical information seeking process and behaviours. This information was then used to construct an EBP lens using familiar healthcare terminology and concepts. Health Science librarians can use this lens as a framework to inform the design and structure of information literacy programmes for clinical staff. Further insight may also be gained by measuring the impact and effectiveness of the lens on information literacy levels and pract...

Optimising the referral process to community-based exercise rehabilitation programmes for cancer survivors: a Delphi study
(2016)

Abstract:
Purpose: Health care professionals (HCPs) are encouraged to consider physical activity (PA) promotion as part of usual care for all cancer patients1. It’s suggested that medical professionals refer patients to external sources for more comprehensive community-based support2. This Delphi study aimed to identify barriers and motivators experienced by HCPs when referring cancer patients to community-based exercise programmes (CBEPs), and strategies to optimise the referral process. Methods: 114 HCPs were invited to complete the round one (R1) online questionnaire. HCPs were asked open-ended questions regarding the motivators and barriers to referral to CBEPs, and strategies to optimise this process. In round two (R2), respondents were asked to rate their level of agreement or disagreement with statements arising from R1. The aim of R2 was to achieve consensus regarding the optimisation of the referral process. Results: A score of 70% (across strongly agree/agree or strongly disagre...

Abstract:
The definitive version is available at http://www3.interscience.wiley.com
AIMS: To determine whether there is an association between Type 2 diabetes mellitus and schizophrenia, independent of medication. METHODS: In this cross-sectional study we performed an oral glucose tolerance test on 38 non-obese white Caucasians who fulfilled the criteria for first-episode drug-naïve schizophrenia, 38 control subjects (matched for age, gender, smoking status, alcohol intake and ethnicity) and 44 first-degree relatives of the patients. RESULTS: The frequency of impaired glucose tolerance (IGT), defined by World Health Organization criteria, was 10.5% (n = 4) in patients with schizophrenia, 18.2% (n = 8) in unaffected relatives and 0.0% in healthy control subjects (chi(2) = 4.22, d.f. = 2, P < 0.05). CONCLUSIONS: The high point prevalence of IGT in never-treated patients and relatives supports either shared environmental or genetic predisposition to IGT. Both patients and their relatives ...

Abstract:
Rheumatoid Arthritis (RA) is the most common Connective Tissue Disease (CTD) and represents an increasing burden on global health resources. Interstitial lung disease (ILD) has been recognised as a complication of RA but its potential for mortality and morbidity has arguably been under appreciated for decades. New studies have underscored a significant lifetime risk of ILD development in RA. Contemporary work has identified an increased risk of mortality associated with the Usual Interstitial Pneumonia (UIP) pattern which shares similarity with the most devastating of the interstitial pulmonary diseases, namely Idiopathic Pulmonary Fibrosis (IPF). In this paper, we discuss recent studies highlighting the associated increase in mortality in RA-UIP. We explore associations between radiological and histopathological features of RA-ILD and the prognostic implications of same. We emphasise the need for translational research in this area given the growing burden of RA-ILD. We highlight t...

Age prediction formulae from radiographic assessment of skeletal maturation at the knee in an Irish population.
(2014)
O'Connor, Jean E; Coyle, Joseph; Bogue, Conor; Spence, Liam D; Last, Jason
Abstract:
Age estimation in living subjects is primarily achieved through assessment of a hand-wrist radiograph and comparison with a standard reference atlas. Recently, maturation of other regions of the skeleton has also been assessed in an attempt to refine the age estimates. The current study presents a method to predict bone age directly from the knee in a modern Irish sample. Ten maturity indicators (A-J) at the knee were examined from radiographs of 221 subjects (137 males; 84 females). Each indicator was assigned a maturity score. Scores for indicators A-G, H-J and A-J, respectively, were totalled to provide a cumulative maturity score for change in morphology of the epiphyses (AG), epiphyseal union (HJ) and the combination of both (AJ). Linear regression equations to predict age from the maturity scores (AG, HJ, AJ) were constructed for males and females. For males, equation-AJ demonstrated the greatest predictive capability (R(2)=0.775) while for females equation-HJ had the stronges...

OBJECTIVES: Older adults are particularly vulnerable to adverse effects from concurrent alcohol and medication use. However, there is limited evidence regarding the prevalence of these adverse outcomes among older adults, and there is a lack of consensus regarding what constitutes an alcohol-interactive medicine. The objective of this study was to develop an explicit list of potentially serious alcohol-medication interactions for use in older adults.

DESIGN: Following a systematic review, review of drug compendia and clinical guidance documents, a two-round Delphi consensus method was conducted.

SETTING: Ireland and the United Kingdom (UK), primary care and hospital setting.

<...

Abstract:

The original book chapter is available at https://www.intechopen.com/

Too few researchers receive adequate pre- or postgraduate training to conduct a rigorous scientific study. In the digital age, new tools are emerging, and the development of distance education could improve this worrying situation. In this context, Health Science e-Training (HSeT), a nonprofit Swiss foundation, has developed new pedagogical concepts and tools under customized online training (COLT). For the ADMIRE Cost network, we have used an article-based e-learning (ABL) tool that allowed the students to learn how to read in depth and critically a scientific article and to rigorously address the problem of scientific reproducibility. The evaluation of the program by the students and the tutors has been quite positive. In conclusion COLT was well adapted to the needs of the ADMIRE Cost Action, a European network in which students from countries separated by thousands of miles can ...

Abstract:
Oral cancer (OC) is a largely asymptomatic disease, resulting in one of the highest mortality rates of any cancer. OC is currently ranked as the sixth most common cancer in the world, according to a recent World Health Organization analysis, and its prevalence is increasing, both in western and developing regions. Depending on the stage of OC, treatment strategies include surgery, radiation therapy and chemotherapy, or a combination thereof. As with numerous other types of cancer, resistance to conventional chemotherapeutic drugs is increasing in oral squamous cell carcinoma (OSCC). The present study aimed to investigate the use of a novel group of compounds, the pyrrolo?1,5?benzoxazepines (PBOXs), as a therapeutic alternative for the treatment of OC. PBOXs are microtubule?targeting agents that are able to induce apoptosis in numerous cancer cell types, thereby preventing tumour cell proliferation. Ca9.22 gingival and TR146 buccal cell lines were used as models for OSSC. Cell viabil...

Abstract:
OBJECTIVE: Glioblastoma multiforme (GBM) is a devastating brain tumor for which there is no cure. Adenoviral-mediated transfer of conditional cytotoxic (herpes simplex virus [HSV] 1-derived thymidine kinase [TK]) and immunostimulatory (Fms-like tyrosine kinase 3 ligand [Flt3L]) transgenes elicited immune-mediated long-term survival in a syngeneic intracranial GBM model in rodents. However, the lack of a large GBM animal model makes it difficult to predict the outcome of therapies in humans. Dogs develop spontaneous GBM that closely resemble the human disease; therefore, they constitute an excellent large animal model. We assayed the transduction efficiency of adenoviral vectors (Ads) encoding beta-galactosidase (betaGal), TK, and Flt3L in J3T dog GBM cells in vitro and in the dog brain in vivo. METHODS: J3T cells were infected with Ads (30 plaque-forming units/cell; 72 h) encoding betaGal (Ad-betaGal), TK (Ad-TK), or Flt3L (Ad-Flt3L). We determined transgene expression by immunocyto...

The development of family therapy in Ireland
(2013)
Carr, Alan
Abstract:
In Ireland family therapy is a small profession, with under 200 registered therapists. The Irish family therapy movement began in the mid-1970s. By 1980 the Family Therapy Network of Ireland in the Republic of Ireland and the Northern Ireland Branch of the UK Association Family therapy had been founded. At present there are three main family therapy training centers in Ireland: two in the south (the Mater University Hospital, affiliated to University College Dublin and Clanwilliam Institute) and one in the north (at Queen’s University Belfast). There is no statutory registration and licensing of family therapists in Ireland. Accredited professional family therapy programs in Ireland are 4-year part-time courses culminating in masters level qualifications. A primary degree in medicine, nursing, psychology, social science or education is a prerequisite for entry. Family therapists in Ireland work in both private practice and the public health service. Most family therapists in the pub...
