<p>BACKGROUND: Multimorbidity has been defined as the co-existence of two or more chronic conditions. It has a profound impact on both the individuals affected and on their use of healthcare services. The limited research to date has focused on its epidemiology rather than the development of interventions to improve outcomes in multimorbidity patients, particularly for patients aged less than 65 years. <p>BACKGROUND: In this paper, we give an overview of methadone treatment in Ireland and outline the rationale for designing an electronic health record (EHR) with extensibility, interoperability and decision support functionality. Incorporating several international standards, a conceptual model applying a problem orientated approach in a hierarchical structure has been proposed for building the EHR.</p> <p>METHODS: A set of archetypes has been designed in line with the current best practice and clinical guidelines which guide the information-gathering process. <p>AkiDwA, the African and Migrant Women’s Network in Ireland, developed this resource as part of a project funded by the Office of the Minister for Integration, examining the health-care needs of women who have undergone female genital mutilation (FGM) in their countries of origin and who now reside in Ireland. <p><strong><em>Background </em></strong></p> <p>Progression of breast cancer despite hormonal and biological treatments continues to pose a therapeutic challenge. Receptor crosstalk is a resistant mechanism that can occur between the estrogen receptor (ER) and the receptor tyrosine kinases, such as HER2, in response to inhibition by anti-endocrine treatments. Briefly, tumours can overactivate growth factor signalling cascades and give rise to an endocrine independent tumour. <p>In this project, we set out to analyse the processes of proliferation, survival and maturation of neuronal precursor cells within the adult mouse hippocampus under physiological and pathological conditions. Injuries to the brain, such as cerebral ischemia, are characterised by neuronal cell death within the affected brain tissue but to date little treatment is available in order to minimise the extent of neuronal damage or to recover the affected tissue. Over-activation of glutamate receptors, termed excitotoxicity, in particular mediated by NMDA (Nmethyl- D-aspartate) receptor subtypes, results in a cascade of events within the cells resulting in apoptotic and necrotic cell death. <p>Collagen-glycosaminoglycan (GAG) scaffolds were initially designed for use in skin regeneration and have since been utilised in cartilage, nerve and muscle tissue engineering. The FDA-approved formulation contains a ratio of 0.5% collagen to 0.044% GAG. However, the composition and stiffness of the collagen-GAG scaffold has never been optimised to sustain a particular cell phenotype or designed with tissue-specific applications in mind. This study sought to investigate the effect of varying the composition and stiffness of the scaffold on the material andstructural properties, in addition to cell behaviour, with a view to developing an optimised collagen-GAG scaffold for use in bone tissueengineering. This discussion document examines some of the legal and practice ramifications of the Data Protection Acts 1988 and 2003 for public health and medical research. The paper was commissioned by the Health Research Board (HRB) subsequent to the expression of general concerns in relation to how the Data Protection Acts would apply to public health and medical research. Many have expressed the concern that a zealously over-regulated and bureaucratic regime would hamper medical research for the wrong reasons. These concerns have been expressed by a number of people in various jurisdictions. <p>Worldwide there is an increasing incidence and prevalence of disability. To provide a wide range of supports to people with disability, the Health Service Executive (HSE) works in partnership with voluntary agencies to provide specialist health and social care services. The integration of this sector has led to an emphasis on joint working of an array of professionals across organisational boundaries as teams and through teamwork. In reality while the need for joint working is an important component of policy, it is something that is not delivered effectively in practice. This organisational development project aims to enhance interagency working by promoting a culture of collaboration and co-ordination of services so that effective support is provided to service users. <p>Global health worker maldistribution affects poor countries and rural areas most adversely, despite their high disease burden. Health workers reject rural areas due to isolation, and lack of facilities. Recommended extrinsic interventions to address rural-urban imbalance are costly and not sustainable in most developing countries. However, some health workers serve in rural areas without such interventions, suggesting existence of strong intrinsic motives for rural practice choice and retention, knowledge of which could be used to select retainable staff. This PhD research, a mixed-methods case study of 50 purposively-selected doctors and nurses retained in 12 Ugandan rural government and private general hospitals for three or more years, sought to find the reasons some qualified health workers get retained in rural areas, and the role of job embeddedness, a construct which predicts employee turnover, in their retention.</p> <p>Rural practice choices were made f...

Although maternal mortality ratios have reduced and significant progress has been made in reducing neonatal morality, many challenges in achieving universal access to maternal, newborn and child health care still exist in Malawi. <p>The case for undertaking policy analysis has been made by a number of scholars and practitioners. However, there has been much less attention given to how to do policy analysis, what research designs, theories or methods best inform policy analysis. This paper begins by looking at the health policy environment, and some of the challenges to researching this highly complex phenomenon. It focuses on research in middle and low income countries, drawing on some of the frameworks and theories, methodologies and designs that can be used in health policy analysis, giving examples from recent studies. <p>Cardiovascular Disease is the number one cause of death in the world, with a mortality rate, globally of 35.1% representing 17.6 million lives lost annually. The majority of these deaths are related to vascular diseases, such as atherosclerotic plaques, which can obstruct blood flow perfusion of organs and tissue causing serious injuries or even death. In the heart, in order to prevent heart failure, blood flow to the tissue must be restored as quickly as possible, ideally via minimally invasive interventions such as stenting or other types of angioplasty as the gold standard. However, this is not always suitable, thus, blood vessel bypass grafts may be required. <p>Outcomes for patients with chronic respiratory diseases remain poor despite the development of novel therapies. In part, this reflects the fact that adherence to therapy is low and clinicians lack accurate methods to assess this issue. Digital technologies hold promise to overcome these barriers to care. For example, algorithmic analysis of large amounts of information collected on health status and treatment use, along with other disease relevant information such as environmental data, can be used to help guide personalised interventions that may have a positive health impact, such as establishing habitual and correct inhaler use. <p>Well-trained and motivated medical staff are essential to a functioning health system. However, Ireland is struggling in this regard. As has been highlighted in numerous studies and reports over the years, Irish non-consultant hospital doctors (NCHDs) are demoralised and dissatisfied with working conditions in Ireland and are leaving in increasing numbers. The other side of the coin is Ireland’s growing reliance on overseas-trained doctors, despite the successful scaling-up of numbers of EU (and therefore Irish) entrants to medical schools.</p>

The findings are based on searches for publicly available national (and any international) curriculum statements of rural and remote practice published by agencies relevant to rural and remote medical practice, both government and non government, across the globe. Environmental stressors are important in the development of psychosis; however, distress may only be pathogenic when it exceeds an individual's ability to cope with it. <p>Following the implementation of the Fottrell report, entry to medical school in Ireland has undergone significant change. Medical school studentship is now awarded based on a combination of points obtained from the final examination of Irish secondary schools (the leaving certificate) combined with HPAT scores (Health Professions Admissions Test). The HPAT is designed to test a candidate's knowledge in several different fields including problem solving skills, logical and non verbal reasoning. A sample HPAT was administered to a test group composed of consultant surgeons, non consultant hospital doctors, and medical students. In addition to its well established role in bone health, initial animal studies have established a relationship between vitamin D deficiency and cardiovascular dysfunction including cardiac hypertrophy, fibrotic change and elevated blood pressure .Emerging evidence notes that age associated arterial stiffness is accelerated in the presence of cardiovascular disease and arterial ageing is a risk factor for adverse CV outcomes.
