Abstract:
<p>Malaria is a parasitic infection which affects approximately 300 million people worldwide causing over one million deaths each year. There are few diseases that have the same impact on human social and economic development than malaria. In areas where malaria is prevalent, the disease can account for up to 40% of public health expenditure and 50% of all hospital visits. It is clear that in order to relieve the burden of malaria novel, cheap, effective treatments need to be investigated, as in this project. In order to suppress the resistance of the disease to drugs, antimalarials with novel mechanisms of action need to be developed.</p> <p>In this research project, plasmodia1 tubulin has been investigated as a potential drug target. <p>BACKGROUND: Multimorbidity affects up to one quarter of primary care populations. It is associated with reduced quality of life, an increased risk of mental health difficulties and increased healthcare utilisation. Functional decline is defined as developing difficulties with activities of daily living and is independently associated with poorer health outcomes. <p>HSE Change Model (2008) was used to improve the delivery of recovery-orientated care in an adult mental health inpatient service. The areas of education on recovery and maintaining recovery on discharge were selected for improvement. A literature review was conducted and found that recovery had common themes and that with training these themes could be translated into practice. Regarding discharge the evidence suggests that when certain interventions are used they can have a dramatic effect on supporting a service-users recovery.</p> <p>During initiation stage, a PEST and SWOT were used to explore the drivers for the change and then a stakeholder analysis was conducted. Mandate was sought from stakeholders who provided buy-in and credibility for the change. During the planning stage focus groups were used to engage staff and put forward a case for change. <p>This thesis describes an implementation plan for a work engagement (Schaufeli & Bakker, 2010) intervention designed for the Irish heath care arena. The hypothesis is based on the principle that when people are happier and more resourced in their workplace, performance is increased, and a higher quality of care is provided. This thesis argues the need for a work engagement intervention and suggests why services and future leaders would benefit from integrating work engagement as a concept into health care organisations.</p> <p>This thesis contains a proposal for the intervention, with drivers for the change and possible resistance being identified. The challenges and current organisational context are discussed and explored. The findings are identified through the process of a systematic literature review exploring work engagement interventions from 2002 to 2014. <p>Irish healthcare has undergone extensive change with spending cuts and a focus on quality initiatives. However, little is known about adverse event occurrence. This thesis assesses the frequency and nature of adverse events in Irish hospitals and compares the results to international adverse events data, using a systematic review methodology and conducting the Irish National Adverse Events Study (INAES).</p> <p>Retrospective patient chart review based on the Harvard Medical Practice Study methodology was undertaken. A random sample of adult in-patient admissions from 2009 was selected from eight hospitals, stratified by region and size, across the Republic of Ireland. 1,574 patient charts (53% female, mean age 54 years) underwent two-stage review (stage-one: nurse review for triggers; stage-two physician review of triggered charts for adverse events) with electronic data capture. <p>The purpose of the organisational development project is to reduce burnout and stress for primary care physiotherapists by implementing a proactive stress management system including a stress control programme. This system is designed to eliminate or minimise psychosocial risks in the workplace and is also restorative by equipping the physiotherapists with practical coping skills. Burnout and stress are the number one occupational reasons for absenteeism in Europe. The moratorium on recruitment in the health care service increased personal and financial demands for patients and staff. The population over 65 will more than double over the next 30 years with evident implications for health service, and other public services, planning and delivery. With this in mind it is inevitable that there will be an increase in the demand for the Home Help Service to maintain those over sixty five as long as possible in their own homes. There is no best way identified by the Health Service Executive to manage change but there is a recognised need to develop strategies to implement change so that the organisation can meet the demands on the services they provide. <p>BACKGROUND: Understanding what determines family size is crucial for programmes that aim to provide family planning services during and after conflicts. Recent research found that development agents in post conflict settings do not necessarily take time to understand the context adequately, translate their context understanding into programming, or adjust programming in the light of changes. They have a right to be treated with respect and to feel safe (Health Service Executive) (HSE 2014b p 13).</p> <p>Striving to provide a high quality, safe and supportive service to older adults in residential care is a consistent challenge and it is critically important as the old age dependency ratio in Ireland is expected to increase from 17.4% in 2012 to 36.3% in 2045 (Central Statistics Office CSO 2012). <p><strong>BACKGROUND:</strong> In spite of bearing a heavier burden of death, disease and disability, there is mixed evidence as to whether Indigenous Australians utilise more or less healthcare services than other Australians given their elevated risk level. <p>Introduction: Following global calls to integrate the delivery of Sexual Reproductive Health (SRH) and HIV/AIDS services efforts have been focused on adopting innovative approaches to respond to the calls. These calls were premised on the belief that delivery of integrated services leads to increase in service availability, access, utilisation as well as efficiency in service delivery. <p>Background</p> <p>Metabolic syndrome has been established as a risk factor for cardiovascular disease and diabetes amongst patients with schizophrenia. Few studies have explored the prevalence rates of metabolic syndrome specific to treatment resistant schizophrenia and the experience of living with the condition.</p> <p>Materials and Methods</p> <p>A sample of 155 patients with treatment resistant schizophrenia was recruited to participate in this mixed methods study. In component 1 of the study the entire sample of patient participants were assessed using International Diabetes Federation (IDF) criteria for metabolic syndrome. In component 2 of the study 26 patient participants took part in semi-structured interviews which explored their experiences of living with metabolic syndrome. <p><strong>OBJECTIVES:</strong> To determine the economic impact of three drugs commonly involved in potentially inappropriate prescribing (PIP) in adults aged ≥65 years, including their adverse effects (AEs): long-term use of non-steroidal anti-inflammatory drugs (NSAIDs), benzodiazepines and proton pump inhibitors (PPIs) at maximal dose; to assess cost-effectiveness of potential interventions to reduce PIP of each drug.</p> <p><strong>DESIGN:</strong> Cost-utility analysis. <p>This change project focused on increasing the awareness and understanding of the Enduring Power of Attorney among members of a regional Mental Health Team, patients and carers. In 2011, 1405 Wardship orders, a provision under the outdated Lunacy Regulations Ireland Act, 1871, were signed in Ireland as opposed to 440 appointed Enduring Powers of Attorney, under Irish Power of Attorney Act 1996. This suggests a probable lack of awareness or understanding and uptake of the provisions for advance care instructions. A pre-audit revealed less than half of a selected population of the Psychiatry of Old Age patients were aware of the Enduring Power of Attorney. <p>Since the amalgamation of mental institutions with acute hospitals there has been an increase in presentations of patients with mental illness to the Emergency Department. The first point of contact for the patient attending the Emergency Department is typically triage. It is the point where emergency care begins with the nurse assessing the patient and assigning a triage category that best suits the patient’s clinical need. Traditionally triage had its origins in assessing patients presenting with a physical injury or illness and did not consider those with a psychological illness. Present-day triage has improved but not to the extent that is required for best practice. Furthermore staff in the Emergency Department may have received no formal training in the speciality that is Mental Health. <p>Many countries have national guidelines for the prevention and control of methicillin-resistant Staphylococcus aureus (MRSA) that are similar in approach. The evidence base for many recommendations is variable, and often, in the drafting of such guidelines, the evidence is either not analysed or not specifically reviewed. Guidelines usually recommend screening and early detection, hand hygiene, patient isolation or cohorting, and decolonization. Although many components of a prevention and control programme appear to be self-evident, e.g. patient isolation, the scientific base underpinning these is poor, and scientifically rigorous studies are required.
