<p><strong>Objectives:</strong> To analyse the policy process that led to changes to the Finance Acts in 2001 and 2002 that gave tax-reliefs to build private hospitals in Ireland.</p> <p><strong>Methods:</strong> Qualitative research methods of documentary analysis and in-depth semi-structured interviews with elites involved in the policy processes, were used and examined through a conceptual framework devised for this research.</p> <p><strong>Results:</strong> This research found a highly politicised and personalised policy making process where policy entrepreneurs, namely private sector interests, had significant impact on the policy process. <p>BACKGROUND: A coordinated response to HIV/AIDS remains one of the 'grand challenges' facing policymakers today. Global health initiatives (GHIs) have the potential both to facilitate and exacerbate coordination at the national and subnational level. Evidence of the effects of GHIs on coordination is beginning to emerge but has hitherto been limited to single-country studies and broad-brush reviews. To date, no study has provided a focused synthesis of the effects of GHIs on national and subnational health systems across multiple countries. <p>BACKGROUND: Global Health Initiatives (GHIs), aiming at reducing the impact of specific diseases such as Human Immunodeficiency Virus (HIV), have flourished since 2000. Amongst these, PEPFAR and GFATM have provided a substantial amount of funding to countries affected by HIV, predominantly for delivery of antiretroviral therapy (ARV) and prevention strategies. Since the need for additional human resources for health (HRH) was not initially considered by GHIs, countries, to allow ARV scale-up, implemented short-term HRH strategies, adapted to GHI-funding conditionality. Such strategies differed from one country to another and slowly evolved to long-term HRH policies. <p>Chest pain is one of the most frequent complaints in medical settings, yet more than half of cases have no detectable cause. Once a cardiac cause has been excluded, patients are typically discharged with a label of non-cardiac chest pain (NCCP). Patients with NCCP have been shown to have poor outcomes in terms of continued symptoms, distress, and continuing concern about heart disease. In addition, NCCP represents a significant burden to health services. The aetiology and management of NCCP is under-researched and poorly understood. This study sought to examine the predictors of persistent pain and health service use for patients in whom a cardiac diagnosis had been excluded. <p><strong>Background: </strong>In examining the sustainability of homecare, gerontological researchers have increasingly recognised how stressful caregiving can be. Indeed, several researchers have postulated that carer stress could increase the risk of institutional care utilisation by care recipients. However, this contention has not been critically analysed. Therefore, this thesis asks to what extent, if any, carer stress influences institutional care utilisation by community-dwelling older people.</p> <p><strong>Methods:</strong> A mixed methods approach was adopted. Study 1 systematically reviewed and meta-analysed the strength of the effect of carer stress on subsequent institutionalisation of community-dwelling older people. <p><strong>Introduction:</strong> Published literature suggests that task shifting is a very affordable way of providing health services in resource constrained settings. However, there is less empirical evidence on its effects on the mainstream tasks and workload of the cadres to whom the tasks are shifted. This thesis examines the consequences of delegating HIV testing and counselling services to Health Surveillance Assistants (HSAs) on their community work, workload and motivation in Malawi.</p> <p><strong>Methods: </strong>Client records for Pentavalent, family planning and HTC for the period 2006-2010, when task shifting using HSAs just started, were collected from eight health facilities in Salima and Mangochi districts. <p>This thesis aimed to identify the evidence base regarding determinants, outcomes, and measures of antihypertensive medication adherence, and to assess the feasibility of monitoring adherence in community pharmacy. The overarching aim is to inform the development of pharmacy-led adherence interventions. Specific aims were to evaluate 1) patient and health-system determinants of antihypertensive adherence, 2) adverse consequences of poor antihypertensive adherence, 3) validity of pharmacy adherence measures, and 4) to assess the feasibility of identifying poor adherence during repeat dispensing.</p> <p>A 12-month prospective cohort study of community-dwelling hypertensive older adults (≥65 years) recruited from 106 community pharmacies was undertaken from 2014 to 2015. <p>Salmonella Typhimurium DT8 was a very rare cause of human illness in Ireland between 2000 and 2008, with only four human isolates from three patients being identified. Over a 19-month period between August 2009 and February 2011, 34 confirmed cases and one probable case of Salmonella Typhimurium DT8 were detected, all of which had an MLVA pattern 2-10-NA-12-212 or a closely related pattern. The epidemiological investigations strongly supported a linkbetween illness and exposure to duck eggs. Moreover, S. Typhimurium with an MLVA pattern indistinguishable (or closely related) to the isolates from human cases, was identified in 22 commercial and backyard duck flocks, twelve of which were linked with known human cases. The serine anti-protease elafin is expressed by monocytes, alveolar macrophages, neutrophils, and at mucosal surfaces and possesses antimicrobial activity. It is also known to reduce lipopolysaccharide-induced neutrophil influx into murine alveoli as well as to abrogate lipopolysaccharide-induced production of matrix metalloprotease 9, macrophage inhibitory protein 2, and tumor necrosis factor-alpha by as-yet unidentified mechanisms. In this report we have shown that elafin inhibits the lipopolysaccharide-induced production of monocyte chemoattractant protein-1 in monocytes by inhibiting AP-1 and NF-kappaB activation. <p>The féminisation of medicine is an international phenomenon referring to increasing numbers of females undertaking medical education and entering the medical workforce. This phenomenon has been a hallmark of the medical profession in Ireland since the 1990s. Associated with this phenomenon are several trends that may have implications for human resources management of the medical workforce. These trends are: distinct variation in the distribution of female doctors across the medical specialties, under-representation of female doctors in senior medical roles, and a greater likelihood of less-than-full-time working among female doctors compared to males. Behavioural and developmental factors mean that adolescents who initiate sexual intercourse early may be at an increased risk of adverse sexual health outcomes at the time of first sex and later in life. In an Irish context, there is insufficient knowledge about the specific correlates of early sexual initiation. This research explores relationships between contextual socio-demographic, health and lifestyle factors and the timing of first sexual intercourse among 15–17-year-olds in Ireland. Multiple regression analysis was carried out in conjunction with Multiple Imputation using data collected through the 2014 Health Behaviour in School-Aged Children Ireland study on a sample of 879 sexually active adolescents. The socio-demographic and lifestyle factors measured were a stronger predictor of age of sexual initiation among girls than boys. <p>BACKGROUND: CAM use is widespread, especially among patients with diabetes. The Gulf States have a high prevalence of diabetes, alongside a long tradition of CAM use. The aim of this study is to establish the prevalence of CAM use among patients with diabetes mellitus in Bahrain and to examine the characteristics of the CAM users.</p> <p>METHODS: A questionnaire was developed and administered to a convenience sample of patients with diabetes (n = 402) above the age of 20 attending two hospital diabetes clinics. Data were analysed using descriptive statistics and non-parametric tests of association.</p> <p>RESULTS: 63% of responders utilized CAM within the previous 12 months. The aim of this study is to assess the level of research activity and capacity for research among primary healthcare professionals in a health authority of over one million people in a mixed urban/rural setting in the West of Ireland.</p> <p>METHODS: A questionnaire, incorporating the R+D Culture Index, was sent to primary healthcare professionals in the HSE Western Region.
