Abstract:
The aim of this audit was to review current practice within a rural mental health service area on the monitoring and documentation of side effects of antipsychotic depot medication. Following a review of the literature on best practice internationally, an evidence based audit tool was adapted. A sample of 60 case files, care plans and prescriptions were audited between January and May 2010. This represented 31% of the total number of service users receiving depot injections in the mental health service region (n=181). The audit results revealed that most service users had an annual documented medical review and a documented prescription. However, only 5 (8%) case notes examined had documentation recorded describing the condition of the injection site and alternation of the injection site was recorded in only 28 (47%) case notes. No case notes examined had written consent to commence treatment recorded, and only 3 (5%) of case notes had documented that information on the depot inject...

Abstract:
The National Council for the Elderly is an advisory body to the Minister for Health on all aspects of ageing and the welfare of the elderly. One of its terms of reference is to advise the Minister on measures to promote the health of the elderly. As one of its contributions towards the realisation of this objective the Council published a report in October 1996 entitled, Mental Disorders in Older Irish People: Incidence Prevalence and Treatment. The report provides a profile of mental disorders in the older Irish population by bringing together in one publication the information which exists on the prevalence, incidence and treatment of mental disorders in older people. There is very little critical analysis of the relatively new policy of Service User Involvement (SUI) in Irish mental-health services (MHS), in spite of lessons from decades of SUI practice internationally. SUI is endorsed by top MHS management as a reform strategy, while for service-users, it is linked to civil and human rights, originating from contestations of current practice in MHS, yet is often confined to a consideration of how better to implement existing services. No study of SUI in Ireland has contrasted and compared the service-user experiences with that of professionals in real world instances of SUI. The research question asks what are the understandings and experiences of service-users and service-providers who become involved in SUI? The research adopts survivor standpoint epistemology in a case study of SUI occurring with a local multidisciplinary team. Mental health nurses are the largest cohort of clinicians working in Irish Mental Health services, and are usually involved in the care of service users experiencing acute psychosis either as in-patients, mental health community facilities or in the service users own home. Some service users place value on communication with nurses as it helps with easing their difficulties (Gilburt, Rose and Slade 2008; Russo and Hamilton 2007, however others report not been listened to, ignored and disempowered (Octwell and Capital Members 2007). Service users’ needs will vary depending on whether they are in an acute, stabilisation, or recovery phase of their mental health presentation. To respond to these changing needs, and as highlighted in A Vision for Change, all stakeholders need to clearly understand both the function of the stand-alone elements of acute community-based secondary mental health care and how these interface with each other to provide a seamless continuum of care. Abstract:
This article describes two creative arts group interventions completed in a CAMHS clinic. Both authors work in the rewarding yet busy and challenging field of adolescent mental health. Following specialist training, we wanted to explore if groupwork using creative arts experiences could support young people attending CAMHS to make discoveries about their own well-being and promote positive mental health. The research results support positive qualitative and quantitative outcomes. We used writing this article as a means to summarise our learning and collate wider research on related topics.

Abstract:
The Health Information and Quality Authority (HIQA) and the Mental Health Commission (MHC) developed these joint National Standards for the Conduct of Reviews of Patient Safety Incidents. These standards were commissioned by the Department of Health and are underpinned by findings from the Chief Medical Officer’s 2014 Report on Perinatal Deaths in HSE Midland Regional Hospital Portlaoise, (1) which recommended the development of national standards on the conduct of reviews of patient safety incidents, following the identification of shortfalls with the current system in Ireland. These Standards provide a framework for best practice in the conduct of reviews of patient safety incidents. They cover the conduct of reviews of patient safety incidents including: review of the incident, implementation of recommendations of the review and sharing the learning from the review.
