Abstract:
In addition to the increasing prevalence of diabetes, our population is growing older and living longer. This survey aimed to determine the care provided to residents with diabetes in Nursing Homes. All 44 Nursing Homes in County Galway were sent postal surveys and 75% (n=33) responded. Of these, 18% (n=6) were Health Service Executive (HSE) Nursing Homes and 82% (n=27) were private Nursing Homes. Nursing Homes had an average of 38.2 residents, 5.2 of whom had diabetes. This equates to a prevalence of 14%. 42% of Nursing Homes had a policy on diabetes management, 56% had access to diabetes care guidelines and 97% had diabetes care plans. Retinal screening, dietetic and chiropody/podiatry services were accessed by 70%, 89% and 97% respectively. A third of residents with diabetes were managed on insulin. Setting: 127 nursing and residential homes in the greater Belfast area of Northern Ireland. Participants: 40 homes in the intervention group (representing 1,366 occupied beds) and 87 homes in the control group (representing 2,751 occupied beds). Background: Older adults living in nursing homes spend the majority of their time inactive. The associated levels of chronic disease places an increasing burden on healthcare systems. Physical activity (PA) interventions delivered through exercise classes may be resource intensive and require specialist staff. The aim of this study is to explore the feasibility and acceptability of a PA programme embedded into the daily lives of older adults living in nursing homes and to examine the preliminary effects of this on physical mobility and quality of life. Methods: A randomised controlled pilot feasibility study, including embedded qualitative and economic components will be carried out. Two randomly selected nursing homes will take part in the study; participants (n=20) in one nursing home will receive a three month PA intervention and participants (n=20) in the other will be a usual care control. <p>Medication management is one of the major roles of a nurse leader in any health care setting particularly in the nursing homes. Evidence suggests that errors do occur at any stage of the medication use process (prescribing, documenting/transcribing, dispensing, administering and monitoring) and these might pose significant risks to older people in nursing homes. Thus, this change project was carried out to reduce the incidence of medication errors, ensure resident’s safety and promote compliance with professional and national standards on medication management. A multiple approach using the PDSA cycle and Kotter’s eight steps change model was adopted to guide the change project. Despite this, there is growing evidence that the management of diabetes in NHs is suboptimal. The reasons for this are complex and poorly understood. This study aimed to identify the current level of diabetes care in NHs using a mixed methods approach.
The nursing managers at all 44 NHs in County Galway in the West of Ireland were invited to participate. A mixed methods approach involved a postal survey, focus group and telephone interviews.
The survey response rate was 75% (33/44) and 27% (9/33) of nursing managers participated in the qualitative research. The reported prevalence of diagnosed diabetes was 14% with 80% of NHs treating residents with insulin. Hypoglycaemia was reported as 'frequent' in 19% of NHs. Coping with a Resident who Wants to go Home Discontentment among residents is seen as one of the most difficult situations facing a staff member. Essentially, they bear the brunt of the resident's discontent in a situation which is not of their making. Some homes deal with this problem by not accepting on a long-term basis, any resident who does not wish to stay. For this reason, in some homes, residents are accepted initially on a trial basis, and if this does not work to the satisfaction of both parties, then the resident is not accepted on a permanent basis. Some staff say that the residents are free to leave if the family are willing to take them. This is the nub of the problem as far as the staff are concerned. They must find a way of dealing with the resident who wants to go home, when their relatives are unable or unwilling to have him or her at home.

Objectives: This study evaluated the sitting room environment of two nursing homes in Ireland, using interactive occupation and social engagement as outcome measures and defining these rooms as occupational spaces. Method: Snapshot observational recordings were made in the main sitting rooms during the periods of time when the rooms were in most active use. Narrative information was also recorded. Results: Residents were more likely to occupy their time in the main sitting room passively, rather than in interactive occupation and social engagement. The nursing home residents with dementia spent approximately 70% of their daily time in the main sitting room areas in states of occupational disengagement. Discussion: Additional insight is provided through pragmatic narrative descriptions of the functioning of the main sitting room environment in terms of interactive occupation and social engagement. Abstract:
The Irish Health Information and Quality Authority (HIQA) published National Quality Standards for Residential Care Settings for Older People in 2009. We reported on experiences of general practitioners (GPs) in Dublin caring for nursing home patients (NHPs) in 2006. We revisit these experiences following publication of HIQAâ s standards. 400 GPs received an anonymous postal survey. Of 204 respondents, 145 (71%) felt NHPs required more contact time and 124 (61%) reported more complex consultations compared to other patients. Only 131 (64%) felt adequately trained in gerontology. 143 (70%) reported access to specialist advice, but only 6 (3%) reported a change in this following HIQA standards. 65 (32%) had witnessed substandard care in a NH, of which 16 (25%) made no report, similar figures to 2006. Abstract:
This paper sets out the main provisions of the 1990 Health (Nursing Homes) Act and attempts to locate the Act as a policy instrument in the wider policy debate on service provision for the elderly. In the analysis and comment which accompany the outline of the Acts contents, some important aspects of implementation are examined and the consequences of particular provisions for the implementation process at health board and national level are given some practical consideration. The paper begins with a discussion of the factors which are shaping international health policy for the elderly. These include: demographic and social structural changes; the emerging critique of the biomedical bias in the care and treatment of the elderly and the new emphasis on more integrated, flexible models of care. The remainder of the paper examines and discusses the objectives and detailed provisions of the Act.

Objectives: Caution is advised when prescribing antipsychotics to people with dementia. This study explored the determinants of appropriate, evidence-based antipsychotic prescribing behaviors for nursing home residents with dementia, with a view to informing future quality improvement efforts and behavior change interventions. Design: Semistructured qualitative interviews based on the Theoretical Domains Framework (TDF). Setting and Participants: A purposive sample of 27 participants from 4 nursing homes, involved in the care of nursing home residents with dementia (8 nurses, 5 general practitioners, 5 healthcare assistants, 3 family members, 2 pharmacists, 2 consultant geriatricians, and 2 consultant psychiatrists of old age) in a Southern region of Ireland. This position paper aims to provide guidance to care of older person’s services and those services which provide care for older persons, who are considering the introduction of such posts by addressing the implications of specialist and advanced nursing practice which relate specifically to care of older persons. The national focus is on the development of the continuum of community services to enable older people to live in their own community in dignity and independence for as long as possible, to restore those who become ill or dependent to as independent a life as possible in their own homes and to provide high quality residential care for older people who can no longer be maintained in independence and dignity at home (DoHC 2000, HSE 2006). The Health Service Executive (HSE) has established an expert advisory group for older persons which will act as a primary source of operational policy and strategic advice in addition to driving integration across the HSE.

Background: Antipsychotic prescribing is prevalent in nursing homes for the management of behavioral and psychological symptoms of dementia (BPSD), despite the known risks and limited effectiveness. Many studies have attempted to understand this continuing phenomenon, using qualitative research methods, and have generated varied and sometimes conflicting findings. To date, the totality of this qualitative evidence has not been systematically collated and synthesized. Aims: To synthesize the findings from individual qualitative studies on decision-making and prescribing behaviors for antipsychotics in nursing home residents with dementia, with a view to informing intervention development and quality improvement in this field. <p>Background: Many countries are developing national audits of stroke care. However, these typically focus on stroke care from acute event to hospital discharge rather than the full spectrum from prevention to long-term care. We report on a comprehensive national audit of stroke care in the community and hospitals in the Republic of Ireland. The findings provide insights into the wider needs of people with stroke and their families, a basis for developing stroke-appropriate health strategies, and a global model for the evaluation of stroke services. Abstract:
We have been asked to advise on a number of issues concerning the provision of nursing home care by health boards prior to their dissolution in 2004. Specifically, we have been asked to advise on the following issues: (i) whether health boards had a duty to provide access to public nursing homes to persons within their functional areas and whether such persons enjoyed a corresponding right to be placed in such homes; (ii) the provision of subventions to persons who· were placed in private nursing homes; and (iii) the potential liability of the State in actions for breach of statutory duty and restitution by persons who were refused access to public nursing homes by health boards.

There are approximately 27,000 patients living in nursing homes in Ireland at present. These are probably our most vulnerable citizens with limited control over their daily lives. The trend over the past five years is for people to be cared for in their own homes by a combination of home help and family for longer. When their dependency reaches a level where this is insufficient, a nursing home is deemed to be the best living situation for them. Therefore, the type of patient I am now seeing in nursing homes is frailer and older than previously. Along with this, these older, frailer patients are increasingly dentate, with huge unmet dental health needs. These patients are high dependency and have poor access to quality oral hygiene and preventive measures. Administering dental treatment to these patients is fraught with difficulty. Background: The importance of appropriate wheelchair and seating provision cannot be underestimated (Gowran 2012). Older adults form the largest user group of wheeled mobility devices (Karmarkar et al 2012) and in nursing homes wheelchairs are often used as the primary means of mobility among residents (Mortenson et al 2012). According to Gavin-Dresechnack et al (2010) eighty percent of residents spend time sitting in a wheelchair every day. Despite this there appears to be no guidelines which influence wheelchair provision in this setting leading to poor use of wheelchairs. Objectives: The aim of this scoping study is to inspect the extent, range and nature of evidence available and factors which influence wheelchair provision in nursing homes. Abstract:
Subvention of Nursing Home Costs Under the Health (Nursing Homes) Act, 1990, persons who are accommodated in nursing homes may be entitled to apply to have the cost of their maintenance subvented by Health Boards. Whether a person qualifies for subvention depends firstly upon an applicant's level of dependency, which takes account of the person's physical and mental condition'. Health Boards classify such dependency into three categories - medium, high or maximum dependency. Following an assessment of dependency a means test is conducted. This determines whether a subvention is payable after account is taken of the means of the applicant. Maximum rates of subvention for each category are set out in regulations made under the Act.

Background This study compared functional outcomes at 3 months after hip fracture surgery between nursing home residents participating in a 10-week outreach rehabilitation program and those receiving usual care. Function, health-related quality of life, and mortality were also compared over 12 months, and outreach program feasibility was assessed. Methods A feasibility trial was undertaken in Canadian nursing homes; of 77 participants, 46 were allocated to Outreach and 31 to Control prior to assessing function or cognition. Outreach participants received 10 weeks of rehabilitation (30 sessions), and Control participants received usual posthospital fracture care in their nursing homes. Abstract:
Long term care (LTC) facilities, also called nursing homes, are often ripe for conflicts which cause stress for residents, their families and staff. This article presents the results of a survey showing how nursing facility administrators in Harris County, Texas, managed conflict within their facilities and how a more positive approach was consistently reflected in how their facilities were rated in US government quality consumer ratings. The concept at the centre of this study, SOS-Semantics of Self in Conflict™, recognises that the degradation of standards due to conflict is not just an event in a nursing care facility. It is a process that is heavily influenced, and in some cases exacerbated, by the way in which facility administrators react to conflict. These reactions have important broader implications for the facility’s best practice retrospectively.
